Busulfan falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Boswellia Serrata interacts with.

Theoretically, Boswellia serrata might increase or decrease the levels and clinical effects of CYP3A4 substrates.
In vitro research shows that Boswellia serrata gum resin inhibits CYP3A4 enzymes. Other in vitro research shows that Boswellia serrata extract inhibits CYP3A4 enzymes at most concentrations, although it may modestly induce enzyme activity at low concentrations.

Here's the gist with busulfan and boswellia. Busulfan is a chemo drug your body partly processes through a liver enzyme called CYP3A4. In test-tube studies, boswellic acids (the active part of this resin) mostly slow that enzyme down, though at very low amounts they might do the opposite. If boswellia blocks CYP3A4, busulfan could clear more slowly and build up, which can mean rougher side effects like very low blood counts, more infections, mouth sores, nausea, or liver strain.

I'll be honest: this is theoretical, based on lab data, not real patients. But busulfan is a high-stakes drug with a narrow safety window, so I wouldn't add boswellia without first checking with your oncology team or pharmacist.

What Is Busulfan Used For?

Brand names: Myleran, Busulfex

Busulfan is used to treat chronic myelogenous leukemia (CML), a cancer of the blood and bone marrow. The specific use depends on the form and product.

Myleran (oral tablet) is used for the palliative treatment of CML — meaning it helps control the disease and relieve symptoms. The intravenous injection forms (Busulfex and Busulfan Injection) are used in combination with another chemotherapy drug, cyclophosphamide, as a conditioning regimen to prepare the body before an allogeneic hematopoietic progenitor cell transplant (a bone marrow or stem cell transplant from a donor) for CML.
