Here is the interaction we found between these two, with the evidence behind it.
CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES
Why this applies to Bisoprolol, Hydrochlorothiazide
Bisoprolol, Hydrochlorothiazide falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Boswellia Serrata interacts with.
In vitro research shows that Boswellia serrata gum resin inhibits CYP2D6 enzymes.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Ziac combines two medicines: bisoprolol, a beta-blocker that slows your heart and lowers blood pressure, and hydrochlorothiazide, a water pill. The bisoprolol part is partly handled by a liver enzyme called CYP2D6. In test-tube studies, boswellic acids from Boswellia appear to slow that same enzyme down. In plain terms, if that happens in real life, bisoprolol could clear a bit more slowly and build up slightly. That might nudge its effects stronger, so you could notice a slower pulse, more tiredness, dizziness, or lightheadedness when standing.
I want to be honest about the strength of this: it's purely theoretical, based only on lab work, not real patient cases. Still, if you take both, it's worth a quick chat with your pharmacist or doctor.

From our pharmacist’s overview on the Bisoprolol, Hydrochlorothiazide page
“This is one of the supplement lists I would read closely before adding anything alongside bisoprolol and hydrochlorothiazide (Ziac). The combination already lowers blood pressure two ways, and most of these interactions push on those same levers, either dropping pressure further or acting like extra diuretics that compound potassium an…”
