Cardiovascular Mender contains six active ingredients: bromelain (an enzyme from pineapple), pancreatin (a digestive enzyme blend), adenosine triphosphate (ATP, a cellular energy molecule), coenzyme Q10 (ubidecarenone), serrapeptase (a protein-digesting enzyme), and nattokinase (a fibrin-digesting enzyme from fermented soy). The product also contains a gelatin capsule as an inactive ingredient.
Does it work?
Moderate evidence
The evidence supporting these ingredients is mixed. Coenzyme Q10 is rated likely effective for CoQ10 deficiency and possibly effective for congestive heart failure, migraines, and diabetic neuropathy.
Adenosine is rated effective for paroxysmal supraventricular tachycardia (a heart rhythm condition) and cardiovascular disease diagnosis. However, bromelain, serrapeptase, pancreatin, and nattokinase all lack sufficient reliable evidence in our data for the cardiovascular and other conditions listed — the data simply doesn't establish them as effective for those uses.
How safe is it?
Well-documented data
Coenzyme Q10 is generally well tolerated; gastrointestinal side effects like nausea and diarrhea occur in less than 1% of users. Bromelain, serrapeptase, and nattokinase are generally well tolerated short-term, but long-term safety data is limited for all three.
Common side effects from bromelain and serrapeptase include stomach upset, nausea, and flatulence. Nattokinase has rare reports of hemorrhage and hypersensitivity reactions in people sensitive to natto.
Adenosine evidence is limited for oral supplements — the prescription injectable form is powerful and causes serious side effects (chest pain, dizziness, heart arrhythmias) when given intravenously, but oral safety hasn't been thoroughly studied. Bromelain and serrapeptase should be avoided during pregnancy; there isn't enough safety data.
Nattokinase should also be avoided in pregnancy. Adenosine and coenzyme Q10 lack sufficient pregnancy safety data — talk with your doctor before use.
During breastfeeding, bromelain, serrapeptase, adenosine, and nattokinase should be avoided due to unknown safety. Coenzyme Q10 safety while breastfeeding hasn't been well studied, so check with your doctor.

Cardiovascular Mender by BioAnue contains six ingredients, several of which interact with medications.
The most serious concern is adenosine triphosphate (ATP) with dipyridamole (Persantine) — a Major interaction that can cause dizziness, fainting, and heart rate problems. Dipyridamole blocks how your body breaks down ATP, so levels can build up dangerously.

Moderate interactions span several ingredient-drug combinations. Bromelain and serrapeptase both may increase bleeding risk with blood thinners (anticoagulants) and antiplatelet drugs like aspirin or warfarin.
Nattokinase carries a similar Moderate risk and also may lower blood pressure too far if you're already taking blood pressure medications. Coenzyme Q10 (ubidecarenone) can reduce how well warfarin works — there are four reported cases — and may add to blood pressure-lowering drugs.
Adenosine may increase the risk of heart block when combined with carbamazepine (Tegretol).
Minor interactions include bromelain with tetracycline antibiotics (which might raise antibiotic levels) and adenosine with caffeine or theophylline (which can block adenosine's effects). Coenzyme Q10 may theoretically reduce the activity of certain chemotherapy drugs that work through oxidative stress, such as alkylating agents.

Nattokinase
Anticoagulant/Antiplatelet Drugs
Nattokinase might increase the risk of bleeding when used with anticoagulant/antiplatelet drugs.
Nattokinase has thrombolytic and anticoagulant effects. There is a case report of intracranial hemorrhage in a patient who added nattokinase to aspirin therapy.
Antihypertensive Drugs
Theoretically, nattokinase might increase the risk of hypotension when used with antihypertensive drugs.
Some clinical research suggests that nattokinase may reduce systolic and diastolic blood pressure.
Ubidecarenone
Alkylating Agents
Coenzyme Q10 has antioxidant effects. Theoretically, this may reduce the activity of chemotherapy drugs that generate free radicals.
Theoretically, antioxidants such as coenzyme Q10 might protect tumor cells from chemotherapeutic agents that work by inducing oxidative stress, such as alkylating agents (e.g., cyclophosphamide) and radiation therapy. The clinical importance of this interaction is unknown.
Warfarin (Coumadin)
Coenzyme Q10 is chemically similar to menaquinone and might have vitamin K-like procoagulant effects, which could decrease the effects of warfarin.
Concomitant use of coenzyme Q10 and warfarin might reduce the anticoagulant effects of warfarin. Four cases of decreased warfarin efficacy thought to be due to coenzyme Q10 have been reported. However, there is some preliminary clinical research that suggests coenzyme Q10 might not significantly decrease the effects of warfarin in patients who have a stable INR.
Antihypertensive Drugs
Theoretically, coenzyme Q10 might have additive effects with antihypertensive drugs.
Some clinical research shows that coenzyme Q10 can significantly lower blood pressure, although other studies have shown conflicting results.
Bromelain
Anticoagulant/Antiplatelet Drugs
Bromelain may have antiplatelet effects and may increase the risk of bleeding if used with anticoagulant or antiplatelet drugs.
There is one case report of a patient experiencing minor bruising while taking bromelain with naproxen. Bromelain is thought to have antiplatelet activity. Whether this interaction is of concern with topical bromelain is unclear. Interference with coagulation of burn wounds has been reported in a patient receiving bromelain-based enzymatic debridement. However, observational research has found that topical bromelain debridement is not associated with increases or decreases in laboratory markers of coagulation when compared with surgical debridement.
Tetracycline Antibiotics
Theoretically, bromelain might increase levels of tetracycline antibiotics.
Laboratory research suggests that bromelain might increase the absorption of tetracycline antibiotics. However, a study in healthy adults reported no difference in tetracycline plasma levels when a 500 mg dose was taken with or without bromelain 80 mg.
Serrapeptase
Anticoagulant/Antiplatelet Drugs
Theoretically, taking serrapeptase with drugs that have antiplatelet or anticoagulant effects might increase the risk of bruising and bleeding.
In vitro research shows that serrapeptase has fibrinolytic activity.
Adenosine Triphosphate
Dipyridamole (Persantine)
Dipyridamole can increase the therapeutic and toxic effects of adenosine.
Dipyridamole decreases the metabolism of adenosine. Intravenous infusion of adenosine in patients who are taking dipyridamole can cause dizziness, bradycardia, and syncope. Dipyridamole should be discontinued for several days prior to a cardiac stress test using adenosine.
Carbamazepine (Tegretol)
Carbamazepine might increase the risk of heart block when used concomitantly with adenosine.
Carbamazepine and adenosine can both cause heart block. Giving them concurrently might produce an additive effect.
Methylxanthines
Methylxanthines are competitive antagonists of adenosine and can block its pharmacologic effects.
The methylxanthines, aminophylline, caffeine, and theophylline, can block the effects of adenosine by acting as competitive antagonists at adenosine cell surface receptors. It is recommended that methylxanthines be avoided for 24 hours prior to cardiac stress tests.
