When used short-term and as directed, cascara is generally well tolerated — most people experience mild belly discomfort or cramping at worst. The real concern is long-term use.
Extended cascara use can lead to serious problems: loss of muscle mass (cachexia), protein in the urine (albuminuria), and blood in the urine (hematuria). Very rarely, if the bark isn't properly aged or prepared, it can cause severe vomiting.
Cascara is not recommended during pregnancy because stimulant laxatives carry risks. While nursing, the active compounds may pass into breast milk, so it's best to avoid it.
