Here's the thing about pairing cascara sagrada with digoxin: it's not a direct chemical clash. The concern is potassium. Cascara is a stimulant laxative, and when used too often or too long, it can flush potassium out of your body. Low potassium (hypokalemia) makes your heart muscle more sensitive to digoxin, which can tip you toward digoxin toxicity.

Since digoxin has a very narrow safety window, that matters. Signs to watch for include nausea, loss of appetite, blurred or yellow-tinged vision, confusion, and an irregular or unusually slow heartbeat. To be fair, this is a theoretical risk based on how these drugs behave, not strong proof. Still, please check with your pharmacist or doctor before using cascara, especially for more than a few days.

Cascara sagrada is a stimulant laxative made from the aged bark of a Pacific Northwest tree, used mainly for short-term relief of constipation. Because it can cause cramping, dehydration, and electrolyte problems—and was removed from over-the-counter laxative products in the U.S. due to safety concerns—it should only be used briefly, if at all, and with guidance from a pharmacist or doctor.

About Digoxin

What Is Digoxin Used For?

Brand names: Lanoxicaps, Lanoxin, Digitek

Digoxin tablets, oral solution, and injections (including Lanoxin brand) are all approved to treat mild to moderate heart failure in adults — improving symptoms like exercise tolerance and reducing hospitalizations, though it does not extend life. It is also approved to control the resting ventricular rate (how fast the lower chambers beat) in adults with chronic atrial fibrillation. The oral solution specifically notes it should not be used for multifocal atrial tachycardia.

In children with heart failure, digoxin is used to strengthen the heart's contractions (increase myocardial contractility). Digoxin is best used alongside a diuretic (water pill) and an ACE inhibitor when possible.
