Modafinil falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Cascara Sagrada interacts with.

Theoretically, cascara sagrada might decrease the effects of CYP3A4 substrates.
In vitro research suggests that cascara sagrada can induce CYP3A4 enzymes, albeit to a much lower degree than rifampin, a known CYP3A4 inducer.

Your body partly clears modafinil using a liver enzyme called CYP3A4. In lab dish studies, cascara sagrada appeared to speed that enzyme up a little, though far less than a strong inducer like rifampin. If that effect happened in real life, your body might break modafinil down a bit faster, which could make it slightly less effective at keeping you awake and alert.

Honestly, this is a theoretical concern based only on test-tube work, so I wouldn't lose sleep over it. Cascara is also only meant for very short-term use. Since modafinil already needs careful handling, just let your pharmacist or doctor know before combining them, especially if your usual alertness seems to slip.

Cascara sagrada is a stimulant laxative made from the aged bark of a Pacific Northwest tree, used mainly for short-term relief of constipation. Because it can cause cramping, dehydration, and electrolyte problems—and was removed from over-the-counter laxative products in the U.S. due to safety concerns—it should only be used briefly, if at all, and with guidance from a pharmacist or doctor.

What Is Modafinil Used For?

Brand names: Provigil, Sparlon, Alertec

Modafinil (also sold as Provigil) is used to improve wakefulness in adults who experience excessive sleepiness due to narcolepsy, obstructive sleep apnea (OSA), or shift work disorder (SWD).

In OSA, modafinil treats the sleepiness — not the underlying airway obstruction itself. It is used alongside standard treatments such as CPAP (a breathing device worn during sleep), not as a replacement for them. If CPAP is appropriate for you, your doctor will want you to use it consistently even while taking modafinil.
