Cascara sagrada has stimulant laxative effects. In some people, cascara sagrada can cause diarrhea. Diarrhea can increase the effects of warfarin, increase international normalized ratio (INR), and increase the risk of bleeding.

Here's the thing to keep in mind if you take warfarin (Coumadin) and you're thinking about cascara sagrada. Cascara is a strong stimulant laxative, and if it gives you significant diarrhea, that can throw off your warfarin balance. When you lose a lot of fluid and your gut empties quickly, your INR can creep up, meaning your blood gets thinner than intended. That raises the chance of bleeding problems, like easy bruising, nosebleeds, bleeding gums, or blood in your urine or stool.

This is a theoretical concern based on how the body works, not strong proof, so don't panic. But warfarin is a medication where small shifts matter. Please don't start or stop anything on your own. Talk with your pharmacist or doctor first, and keep up with your regular INR checks.

In vitro research suggests that cascara sagrada can induce CYP3A4 enzymes, albeit to a much lower degree than rifampin, a known CYP3A4 inducer.

Here's the gist with warfarin and cascara sagrada. Lab studies hint that cascara might gently speed up a liver enzyme (CYP3A4) that helps clear some drugs. In theory, that could make a little more of your warfarin get broken down, which might nudge your blood thinning down and your clotting risk up. I want to be honest, though: this signal comes only from test-tube research, the effect was far weaker than known inducers, and it's rated minor. So it may not amount to much in real life.

Still, warfarin is a finicky medication where small shifts can change your INR. Cascara's bigger, more reliable risk is the diarrhea and fluid or electrolyte loss it causes, which can also throw off warfarin control. Please loop in your pharmacist or doctor before pairing them.

Cascara sagrada is a stimulant laxative made from the aged bark of a Pacific Northwest tree, used mainly for short-term relief of constipation. Because it can cause cramping, dehydration, and electrolyte problems—and was removed from over-the-counter laxative products in the U.S. due to safety concerns—it should only be used briefly, if at all, and with guidance from a pharmacist or doctor.

What Is Warfarin Used For?

Brand names: Coumadin, Panwarfin, Sofarin

Warfarin sodium tablets and Jantoven tablets are both used to prevent and treat venous thrombosis (blood clots in the veins) and pulmonary embolism (a clot in the lungs). They are also used to prevent clot-related complications in people with atrial fibrillation (an irregular heart rhythm) or who have had a heart valve replaced.

In people who have had a heart attack, warfarin is used to reduce the risk of death, another heart attack, stroke, or other clotting events. It does not dissolve an existing clot, but it helps prevent the clot from growing and stops new dangerous clots from forming.
