Here's the gist when you pair cascara sagrada with hydrocortisone: both can lower your body's potassium. Stimulant laxatives like cascara pull potassium out through your gut, especially with long-term use, and oral hydrocortisone (Cortef) can nudge potassium down too. Stack them together and your potassium could dip enough to leave you feeling tired, weak, crampy, or noticing an irregular heartbeat.

I'll be honest, this concern is mostly theoretical, based on how the two work rather than strong studies, so don't panic. A short dose of cascara is far less worrisome than using it for weeks. If you take hydrocortisone regularly and are reaching for cascara, just check with your pharmacist or doctor first so they can keep an eye on your potassium.

Hydrocortisone also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Theoretically, cascara sagrada might decrease the effects of CYP3A4 substrates.
In vitro research suggests that cascara sagrada can induce CYP3A4 enzymes, albeit to a much lower degree than rifampin, a known CYP3A4 inducer.

Here's the gist: oral hydrocortisone (Cortef) gets processed in your liver by an enzyme called CYP3A4. Some lab studies hint that cascara sagrada might gently speed that enzyme up. If that happened in real life, your body could clear the steroid a little faster, which in theory might make it work slightly less well. For someone relying on hydrocortisone to replace a missing hormone or to calm inflammation, that could mean symptoms creeping back, like more joint pain, swelling, or fatigue.

I want to keep this in perspective. This is only test-tube evidence, the effect was much weaker than known strong inducers, and it's rated a minor, possible concern. Skin creams aren't affected. Still, if you plan to use cascara, mention it to your pharmacist or doctor first.

Cascara sagrada is a stimulant laxative made from the aged bark of a Pacific Northwest tree, used mainly for short-term relief of constipation. Because it can cause cramping, dehydration, and electrolyte problems—and was removed from over-the-counter laxative products in the U.S. due to safety concerns—it should only be used briefly, if at all, and with guidance from a pharmacist or doctor.

What Is Hydrocortisone Used For?

Brand names: Cortef, Hydrocortone, Carmol HC cream, Hydrocortisone Iodoquinol Cream, Vytone Cream, Massengil Medicated Towelettes, Dioderm, Efcortelan Cream, Mildison, Alphaderm, Calmurid HC, Efcortelan Ointment, Texacort, Alkindi Sprinkle

Hydrocortisone is used to calm skin inflammation and relieve itching caused by conditions that respond to corticosteroid treatment. In adults, it treats a broad range of inflammatory skin conditions. In children as young as 3 months old, it is specifically approved for mild to moderate atopic dermatitis (eczema).

It is also used in rectal foam form to treat inflammation of the lower bowel, such as ulcerative proctitis.
