Here's the thing about pairing cascara sagrada with cyclothiazide: both can lower your potassium. Cyclothiazide is a thiazide-type water pill, and one of its well-known side effects is flushing potassium out through your kidneys. Cascara, used as a stimulant laxative, can push potassium out through your gut, especially with frequent or long-term use. Stack the two and you may drop your potassium lower than either would alone.

This is a theoretical concern based on how these two work, not something proven in studies, so don't panic. But low potassium can cause muscle cramps, weakness, fatigue, or an irregular heartbeat. An occasional dose is less worrisome than regular use. Please check with your pharmacist or doctor before combining them.

Cascara sagrada is a stimulant laxative made from the aged bark of a Pacific Northwest tree, used mainly for short-term relief of constipation. Because it can cause cramping, dehydration, and electrolyte problems—and was removed from over-the-counter laxative products in the U.S. due to safety concerns—it should only be used briefly, if at all, and with guidance from a pharmacist or doctor.
