Fluticasone falls under CORTICOSTEROIDS — the group Cascara Sagrada interacts with.

Theoretically, cascara sagrada might increase the risk of hypokalemia when taken with corticosteroids.

Here's the gist with these two together. Cascara sagrada is a stimulant laxative, and using it for a long stretch can flush potassium out of your body and leave you low (a state called hypokalemia). Fluticasone is a corticosteroid, and steroids can nudge potassium down too. Stack the two and, in theory, you could end up lower than either would cause alone.

Now, a big honest caveat: this is a theoretical concern based on how the two work, not something well documented in real patients. It's also less of a worry with inhaled or nasal fluticasone (Flovent, Flonase), since very little reaches your bloodstream. If you ever feel muscle cramps, weakness, or a fluttery heartbeat, or you plan to use cascara more than briefly, check with your pharmacist or doctor first.

Fluticasone also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Theoretically, cascara sagrada might decrease the effects of CYP3A4 substrates.
In vitro research suggests that cascara sagrada can induce CYP3A4 enzymes, albeit to a much lower degree than rifampin, a known CYP3A4 inducer.

Here's the gist: your body clears fluticasone using a liver enzyme called CYP3A4. There's some early lab work suggesting cascara sagrada can nudge that enzyme to work a little faster, which would clear fluticasone out more quickly. In plain terms, that could mean slightly less medicine sticking around to do its job, whether you're using Flonase for nasal allergies or Flovent for asthma.

Honestly, I wouldn't lose sleep over this one. The evidence is only from test-tube studies, the effect was far weaker than known strong inducers, and inhaled or nasal fluticasone acts mostly right where you spray it. Still, cascara is a harsh laxative best used briefly. If you're leaning on it regularly, check in with your pharmacist or doctor first.

Cascara sagrada is a stimulant laxative made from the aged bark of a Pacific Northwest tree, used mainly for short-term relief of constipation. Because it can cause cramping, dehydration, and electrolyte problems—and was removed from over-the-counter laxative products in the U.S. due to safety concerns—it should only be used briefly, if at all, and with guidance from a pharmacist or doctor.
