CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES
Why this applies to Everolimus
Everolimus falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Cascara Sagrada interacts with.
In vitro research suggests that cascara sagrada can induce CYP3A4 enzymes, albeit to a much lower degree than rifampin, a known CYP3A4 inducer.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the situation with these two together. Your body relies on a liver enzyme called CYP3A4 to clear everolimus out of your system. Some early lab studies hint that cascara sagrada might gently speed that enzyme up. If it does, your body could break down everolimus a little faster, which in theory means slightly lower levels and possibly a weaker effect against your cancer or tumor.
I want to keep this in perspective, though. This concern comes only from test-tube research, and the effect on the enzyme looked much weaker than known strong inducers like rifampin. We don't have real patient evidence that it actually matters. Still, everolimus is a serious medication where steady levels count, so please loop in your oncologist or pharmacist before using cascara sagrada rather than changing anything on your own.

Cascara sagrada is a stimulant laxative made from the aged bark of a Pacific Northwest tree, used mainly for short-term relief of constipation. Because it can cause cramping, dehydration, and electrolyte problems—and was removed from over-the-counter laxative products in the U.S. due to safety concerns—it should only be used briefly, if at all, and with guidance from a pharmacist or doctor.
About Everolimus
What Is Everolimus Used For?
Brand names: Certican, Afinitor, Zortress, Afinitor Disperz
Everolimus is used to treat several serious conditions. In cancer, it is prescribed for advanced hormone receptor-positive, HER2-negative breast cancer (in postmenopausal women), certain neuroendocrine tumors of the pancreas, GI tract, or lung, and advanced renal cell carcinoma (kidney cancer) when other treatments have stopped working.
It is also used for people with tuberous sclerosis complex (TSC) — a genetic condition that causes benign tumors to grow throughout the body. In TSC, it treats kidney tumors called angiomyolipomas, brain tumors called SEGA, and seizures. In kidney transplant patients, it helps prevent the body from rejecting the new organ.
