Here's the thing to keep an eye on with this combo. Both cascara sagrada and prednisolone can pull your potassium levels down. Cascara is a stimulant laxative, so frequent or long-term use can flush potassium out through the gut. Prednisolone, a steroid, can cause your body to waste potassium too. Stack the two, and that drop in potassium can add up.

Low potassium can show up as muscle cramps, weakness, fatigue, or an irregular heartbeat. Now, this is a theoretical concern based mostly on how these two work, not on strong studies, so don't panic. A single short course is unlikely to be a problem for most folks. But if you use cascara often or take prednisolone for a while, please check in with your pharmacist or doctor so they can keep tabs on your potassium.

Prednisolone also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Theoretically, cascara sagrada might decrease the effects of CYP3A4 substrates.
In vitro research suggests that cascara sagrada can induce CYP3A4 enzymes, albeit to a much lower degree than rifampin, a known CYP3A4 inducer.

Here's the gist for these two together. Your body clears prednisolone partly through a liver enzyme called CYP3A4. Some early lab research hints that cascara sagrada may gently speed up that enzyme, which could move prednisolone out of your system a little faster. In plain terms, that might make your steroid work a touch less than expected, so a flare of whatever you're treating (joint pain, asthma, inflammation, or an allergic reaction) could creep back.

That said, this is very weak, theory-only evidence from test-tube work, and the effect looked small. For most people taking cascara briefly, it's unlikely to cause a real problem. Still, since steroids matter for serious conditions, just give your pharmacist or doctor a heads-up before pairing them.

Cascara sagrada is a stimulant laxative made from the aged bark of a Pacific Northwest tree, used mainly for short-term relief of constipation. Because it can cause cramping, dehydration, and electrolyte problems—and was removed from over-the-counter laxative products in the U.S. due to safety concerns—it should only be used briefly, if at all, and with guidance from a pharmacist or doctor.

Prednisolone is prescribed for many different conditions where inflammation or an overactive immune system is causing harm. These range from severe allergic reactions and asthma to certain skin diseases, blood disorders, eye problems, and some cancers.

It is also used to support the body when the adrenal glands aren't making enough of their own steroids, and to manage flares of conditions like ulcerative colitis, multiple sclerosis, and nephrotic syndrome (a kidney condition causing protein loss in the urine).
