Here's the thing to keep an eye on with this combo. Hydrochlorothiazide is a water pill that already nudges your kidneys to flush out potassium along with extra salt and fluid. Cascara sagrada, especially with regular or prolonged use, can pull potassium out too through its laxative action and the fluid loss it causes. Stack the two and your potassium could dip lower than you'd like.

Low potassium (hypokalemia) can show up as muscle cramps, weakness, fatigue, or an irregular heartbeat. I'll be honest, this is a theoretical concern based on how both work, not something proven in big studies, and a one-time short use is unlikely to cause trouble. Still, since both lower potassium, check with your pharmacist or doctor before pairing them, particularly if you'd be using cascara for more than a few days.

Cascara sagrada is a stimulant laxative made from the aged bark of a Pacific Northwest tree, used mainly for short-term relief of constipation. Because it can cause cramping, dehydration, and electrolyte problems—and was removed from over-the-counter laxative products in the U.S. due to safety concerns—it should only be used briefly, if at all, and with guidance from a pharmacist or doctor.

What Is Hydrochlorothiazide Used For?

Brand names: Esidrix, Hydro, Microzide, HydroDiuril, Hydro-D, Oretic, Aquazide H, Aquazide, HCTZ

Hydrochlorothiazide treats high blood pressure (hypertension) in adults and children, either alone or combined with other blood pressure medicines. Lowering blood pressure reduces the risk of strokes and heart attacks.

It also treats edema (fluid buildup) linked to congestive heart failure, hepatic cirrhosis (liver scarring), and kidney conditions including nephrotic syndrome. Hydrochlorothiazide tablets are additionally indicated for edema caused by corticosteroid or estrogen therapy, and edema from acute glomerulonephritis or chronic renal failure. Inzirqo (the oral suspension) is specifically approved for both hypertension and edema in adult and pediatric patients.
