Erythromycin Stearate falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Cascara Sagrada interacts with.

Theoretically, cascara sagrada might decrease the effects of CYP3A4 substrates.
In vitro research suggests that cascara sagrada can induce CYP3A4 enzymes, albeit to a much lower degree than rifampin, a known CYP3A4 inducer.

Here's the short version for these two together. Your body uses a liver enzyme called CYP3A4 to help process erythromycin, and lab studies hint that cascara sagrada might gently rev that enzyme up. If that happened in real life, your body could clear the antibiotic a little faster, which in theory might leave less of it around to fight your infection.

I want to be honest about how weak this signal is. It comes only from test-tube research, the effect was far milder than known enzyme boosters, and it's rated a minor, theoretical concern. Erythromycin is also itself a strong CYP3A4 blocker, so the practical picture is murky. The bigger everyday worry with cascara is cramping, diarrhea, and dehydration. Since you want your antibiotic to fully work, just mention you're using cascara to your pharmacist or doctor.

Cascara sagrada is a stimulant laxative made from the aged bark of a Pacific Northwest tree, used mainly for short-term relief of constipation. Because it can cause cramping, dehydration, and electrolyte problems—and was removed from over-the-counter laxative products in the U.S. due to safety concerns—it should only be used briefly, if at all, and with guidance from a pharmacist or doctor.

About Erythromycin

What Is Erythromycin Used For?

Brand names: Erythro-S, Erythrocin, Ethril

Erythromycin treats bacterial infections caused by susceptible organisms. Oral and intravenous forms are used for upper and lower respiratory tract infections, skin and soft-tissue infections, pertussis (whooping cough), Legionnaires' disease, sexually transmitted infections (gonorrhea, syphilis, chlamydia), listeriosis, and other conditions. Erythrocin Lactobionate IV and erythromycin lactobionate injection are specifically for patients who cannot take oral medication or who need rapid, high blood levels due to severe infection — IV therapy should be switched to oral as soon as appropriate.

Erythromycin topical solution 2% and Ery 2% Pads (swabs) are indicated only for the topical treatment of acne vulgaris (acne) and are applied directly to the skin — they are not used for internal infections.
