Medroxyprogesterone Acetate falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Cascara Sagrada interacts with.

Theoretically, cascara sagrada might decrease the effects of CYP3A4 substrates.
In vitro research suggests that cascara sagrada can induce CYP3A4 enzymes, albeit to a much lower degree than rifampin, a known CYP3A4 inducer.

Your body uses a liver enzyme called CYP3A4 to help clear medroxyprogesterone. There's some early lab research hinting that cascara sagrada may gently speed up that enzyme. If that happened in real life, your body might break down the progestin a little faster, which could in theory make it work a bit less well (for example, less reliable control of irregular bleeding or periods).

Here's the reassuring part: this is rated minor and comes only from test-tube studies, and the effect was far weaker than known enzyme boosters. So I wouldn't lose sleep over it. Since cascara is also a strong laxative that's best used only briefly, please check with your pharmacist or doctor before pairing the two.

Cascara sagrada is a stimulant laxative made from the aged bark of a Pacific Northwest tree, used mainly for short-term relief of constipation. Because it can cause cramping, dehydration, and electrolyte problems—and was removed from over-the-counter laxative products in the U.S. due to safety concerns—it should only be used briefly, if at all, and with guidance from a pharmacist or doctor.

What Is Medroxyprogesterone Used For?

Brand names: Proclim, Cycrin, Curretab, Depo-Provera CI

Medroxyprogesterone is used for different conditions depending on the product form. The injectable forms — including Depo-Provera and intramuscular medroxyprogesterone acetate injectable suspension — are used to prevent pregnancy. Depo-subQ Provera 104 (the subcutaneous injection) is also approved to manage pain associated with endometriosis.

The oral tablets — sold under the brand Provera and as generic medroxyprogesterone acetate tablets — treat secondary amenorrhea (periods that have stopped) and abnormal uterine bleeding caused by hormonal imbalance. The tablets are also used to prevent endometrial hyperplasia (abnormal thickening of the uterine lining) in postmenopausal women with a uterus who are taking daily conjugated estrogens.
