Here's the thing to keep an eye on with this pair. Methyclothiazide is a thiazide water pill, and one of its known effects is lowering your potassium level over time. Cascara sagrada, as a stimulant laxative, can also push potassium out of your body, especially if you use it for more than a few days. Stack the two together and you could drop your potassium lower than either would alone.
Low potassium can leave you feeling tired, crampy, or weak, and in some cases cause an irregular heartbeat. To be clear, this concern is based on how both products work, not on strong studies, so it's a theoretical caution rather than a proven danger. A short, one-time use is far less worrisome than regular laxative use. Still, it's worth a quick chat with your pharmacist or doctor before combining them, and they may want to check your potassium.

Cascara sagrada is a stimulant laxative made from the aged bark of a Pacific Northwest tree, used mainly for short-term relief of constipation. Because it can cause cramping, dehydration, and electrolyte problems—and was removed from over-the-counter laxative products in the U.S. due to safety concerns—it should only be used briefly, if at all, and with guidance from a pharmacist or doctor.

Methyclothiazide is used to treat high blood pressure. Methyclothiazide is also used to treat edema (fluid retention; excess fluid held in body tissues) caused by various medical problems, including heart, kidney, and liver disease and to treat edema caused by using certain medications including estrogen and corticosteroids. Methyclothiazide is in a class of medications called diuretics ('water pills'). It works by causing the kidneys to get rid of unneeded water and salt from the body into the urine.

Damage to these organs may cause heart disease, a heart attack, heart failure, stroke, kidney failure, loss of vision, and other problems. These changes include eating a diet that is low in fat and salt, maintaining a healthy weight, exercising at least 30 minutes most days, not smoking, and using alcohol in moderation.
