Tramadol falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Cascara Sagrada interacts with.

Theoretically, cascara sagrada might decrease the effects of CYP3A4 substrates.
In vitro research suggests that cascara sagrada can induce CYP3A4 enzymes, albeit to a much lower degree than rifampin, a known CYP3A4 inducer.

Here's the gist for tramadol. Lab studies hint that cascara sagrada might gently speed up CYP3A4, one of the liver enzymes that handles tramadol. If that enzyme works a little faster, your body could clear tramadol somewhat quicker, which in theory might soften its pain-relieving effect. Tramadol is a bit unusual though, since it's partly a prodrug and also leans on a separate enzyme (CYP2D6) for much of its painkilling punch, so the real-world impact here is uncertain.

The good news: this is rated minor and is based only on test-tube research, with cascara nudging the enzyme far weaker than strong inducers do. I wouldn't lose sleep over it. Still, if your pain control seems to slip, or you're using cascara more than briefly, check in with your pharmacist or doctor.

Cascara sagrada is a stimulant laxative made from the aged bark of a Pacific Northwest tree, used mainly for short-term relief of constipation.
