Cascara sagrada has stimulant laxative effects. In some people, cascara sagrada can cause diarrhea. Diarrhea can increase the effects of warfarin, increase international normalized ratio (INR), and increase the risk of bleeding.

Here's the connection between these two. Cascara sagrada is a strong stimulant laxative, and if it gives you a bout of diarrhea, that upset gut can actually nudge your warfarin to work a little harder. The thinking is that diarrhea can shift how much vitamin K your body absorbs and how warfarin behaves, which may push your INR higher and raise your chance of bleeding.

I want to be honest that this is a theoretical concern based on how the body works, not strong proof, so don't panic. But since warfarin is a medication where small changes matter, watch for warning signs like easy bruising, nosebleeds, bleeding gums, or unusually dark stools. If you're thinking about cascara, please check with your pharmacist or doctor first so they can keep an eye on your INR.

Why this applies to Warfarin

Warfarin falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Cascara Sagrada interacts with.

Theoretically, cascara sagrada might decrease the effects of CYP3A4 substrates.
In vitro research suggests that cascara sagrada can induce CYP3A4 enzymes, albeit to a much lower degree than rifampin, a known CYP3A4 inducer.

Here's the honest picture with these two. Warfarin keeps your blood thin enough to prevent clots, but not so thin that you bleed easily, so it has very little wiggle room. Cascara sagrada has shown, in lab dish studies only, that it might gently speed up one of the liver enzymes (CYP3A4) that helps clear certain drugs. In theory that could nudge warfarin levels down a bit, which might make it slightly less protective against clots.

In real life this effect looks weak and isn't well proven in people, so I wouldn't lose sleep over it. The bigger concern is that cascara causes diarrhea and fluid and electrolyte loss, which on its own can throw off your INR. If you're considering it, please check with your pharmacist or doctor first and keep up your regular INR checks.

Cascara sagrada is a stimulant laxative made from the aged bark of a Pacific Northwest tree, used mainly for short-term relief of constipation. Because it can cause cramping, dehydration, and electrolyte problems—and was removed from over-the-counter laxative products in the U.S. due to safety concerns—it should only be used briefly, if at all, and with guidance from a pharmacist or doctor.

What Is Warfarin Used For?

Brand name: Warfarin

Warfarin sodium tablets and Jantoven tablets are both used to prevent and treat venous thrombosis (blood clots in the veins) and pulmonary embolism (a clot in the lungs). They are also used to prevent clot-related complications in people with atrial fibrillation (an irregular heart rhythm) or who have had a heart valve replaced.

In people who have had a heart attack, warfarin is used to reduce the risk of death, another heart attack, stroke, or other clotting events. It does not dissolve an existing clot, but it helps prevent the clot from growing and stops new dangerous clots from forming.
