Here's the concern with these two together. Mannitol (Osmitrol) is an osmotic diuretic that pulls extra fluid into your urine, and along the way it can flush out potassium. Cascara sagrada is a stimulant laxative, and with regular or prolonged use it can also drain potassium through the gut. Stack them up and you could end up with low potassium (hypokalemia), which may show up as muscle cramps, weakness, fatigue, or an irregular heartbeat.

I'll be honest with you, this is a theoretical worry based on how both work, not something proven by strong studies. Cascara isn't recommended for routine use anyway. Mannitol is usually given short-term in a hospital, so overlap is uncommon, but please check with your pharmacist or doctor before combining them.
