Rifampin falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Cascara Sagrada interacts with.

Theoretically, cascara sagrada might decrease the effects of CYP3A4 substrates.
In vitro research suggests that cascara sagrada can induce CYP3A4 enzymes, albeit to a much lower degree than rifampin, a known CYP3A4 inducer.

Here's a bit of irony with this pairing: rifampin is actually one of the strongest natural "speed-up" signals for the liver enzyme CYP3A4. Cascara sagrada appears, in lab dish studies only, to nudge that same enzyme in the same direction, but very weakly compared to rifampin itself. So in theory, cascara could push your body to clear certain medications a little faster, lowering their levels.

For rifampin specifically, this is really nothing to lose sleep over. The signal is theoretical and comes only from test-tube data, not from real patients, and rifampin already dominates that pathway on its own. The bigger practical concern is that cascara is a harsh stimulant laxative that can cause cramping, dehydration, and electrolyte loss. If you're tempted to use it, check with your pharmacist or doctor first.

Cascara sagrada is a stimulant laxative made from the aged bark of a Pacific Northwest tree, used mainly for short-term relief of constipation. Because it can cause cramping, dehydration, and electrolyte problems—and was removed from over-the-counter laxative products in the U.S. due to safety concerns—it should only be used briefly, if at all, and with guidance from a pharmacist or doctor.

About Rifampin

What Is Rifampin Used For?

Brand names: Rifadin, Rimactane, Rifadin Injection, Rofactor

Rifampin treats all forms of tuberculosis (TB). It is almost always used in combination with other antibiotics — your doctor will build a multi-drug regimen tailored to your infection. Treatment usually runs for at least 6 months total.

Rifampin is also used to clear Neisseria meningitidis bacteria from the nose and throat in people who carry the germ without being sick themselves. It is not used to treat active meningococcal disease. The intravenous (IV) forms — Rifadin IV, Rifadin IV NOVAPLUS, and the injectable form of Rifampin — are specifically indicated when a patient cannot take the drug by mouth.
