Omeprazole falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Cascara Sagrada interacts with.

Theoretically, cascara sagrada might decrease the effects of CYP3A4 substrates.
In vitro research suggests that cascara sagrada can induce CYP3A4 enzymes, albeit to a much lower degree than rifampin, a known CYP3A4 inducer.

Here's the gist for omeprazole and cascara sagrada. Omeprazole is a regular drug (not a prodrug), and one of the liver pathways that helps clear it is CYP3A4. Lab studies hint that cascara sagrada may mildly speed up that enzyme, which in theory could clear omeprazole a little faster and slightly soften its acid-blocking effect. If that happened, you might notice your heartburn or reflux creeping back.

The good news: this is only based on test-tube work, the effect was much weaker than known strong inducers, and it's rated minor. So it's mostly theoretical, not something to lose sleep over. Since cascara is a harsh laxative meant only for very short use, just loop in your pharmacist or doctor before combining them.

Cascara sagrada is a stimulant laxative made from the aged bark of a Pacific Northwest tree, used mainly for short-term relief of constipation. Because it can cause cramping, dehydration, and electrolyte problems—and was removed from over-the-counter laxative products in the U.S. due to safety concerns—it should only be used briefly, if at all, and with guidance from a pharmacist or doctor.

About Omeprazole

What Is Omeprazole Used For?

Brand names: Prilosec, Zegerid, Losec

Omeprazole is used to treat a range of conditions where stomach acid causes problems. These include heartburn and acid reflux (GERD), erosive esophagitis (damage to the esophagus from acid), and stomach or duodenal ulcers. It can also be used alongside antibiotics to help get rid of an H. pylori infection — a bacteria that causes ulcers.

In more serious cases, omeprazole is used to manage conditions where the stomach produces far too much acid, such as pathologic hypersecretory conditions (rare medical disorders that cause extreme acid overproduction). It is approved for adults and children one year of age and older for GERD and erosive esophagitis.
