That becomes risky if you're also taking corticosteroids, water pills (diuretics), or digoxin — each of those can lower potassium too, and the combined effect could lead to dangerously low levels or even heart rhythm problems with digoxin.

Cascara sagrada can also increase bleeding risk if you take warfarin, a blood thinner, because the laxative effect can cause diarrhea, which makes warfarin work harder and raises your bleeding risk. If you use other stimulant laxatives at the same time, you'll compound fluid and electrolyte losses.
There's also a minor concern that cascara sagrada may slightly reduce how well your body processes certain medications that rely on a liver enzyme called CYP3A4.
Additionally, in vitro research suggests that cascara sagrada can induce CYP3A4 enzymes, albeit to a much lower degree than rifampin, a known CYP3A4 inducer. Altogether, these interactions span 746 individual medications.
