How safe is it?
Well-documented data
Chinese Peony is generally well tolerated for short-term use, though high-quality safety data are limited. The most common side effects are gastrointestinal — abdominal distension, anorexia, diarrhea, nausea, and gastrointestinal discomfort — reported in some patients; diarrhea occurred in about 5% of people taking a related form (TGP) at high doses versus 1% on placebo.
Avoid peony during pregnancy, as safety data are insufficient. There is not enough reliable information to consider it safe while breastfeeding.
Atractylodes is often used traditionally but has limited human safety data; use cautiously. Common side effects are allergic reaction, dry mouth, and nausea.
Avoid it during pregnancy and breastfeeding due to insufficient safety information. Bitter Melon is generally well tolerated as food but may lower blood sugar and cause stomach upset at supplement doses.
The most common side effects are gastrointestinal — heartburn, nausea, vomiting, diarrhea, constipation, and abdominal discomfort — occurring in roughly 3% to 16% of people in studies. Rare serious effects include hypoglycemic coma and seizures in children.

Cell Saver by Allergy Research Group does have documented interactions with medications, primarily from its Chinese Peony and Bitter Melon ingredients.
The most serious interaction involves Chinese Peony and blood thinners (anticoagulant and antiplatelet drugs) — peony may increase bleeding risk through anticoagulant and antiplatelet effects shown in lab research. There's also a case report of a patient on warfarin who experienced dangerous bleeding after switching Chinese herbal brands containing peony.

Additionally, Bitter Melon can lower blood sugar (hypoglycemia) and may have an additive effect with diabetes medications, raising the risk of low blood sugar episodes. Peony theoretically interacts with drugs your liver breaks down via CYP1A2 and CYP3A4 pathways — including clozapine — by slowing their clearance and raising their levels.
Bitter Melon may also increase levels of certain cancer drugs (pazopanib) and other medications that depend on a gut pump called P-glycoprotein for absorption.

Chinese Peony (root) extract
Anticoagulant/Antiplatelet Drugs
Theoretically, combining peony with anticoagulant or antiplatelet drugs might increase the risk of bleeding.
In vitro research suggests that peony might have antiplatelet, anticoagulant, and antithrombotic effects.
Clozapine (Clozaril)
Theoretically, peony might increase the levels and clinical effects of clozapine.
In vitro research shows that peony suppresses the metabolism of clozapine via weak-to-moderate inhibitory effects on cytochromes P450 (CYP) 1A2 and CYP3A4. This effect has not been reported in humans.
Contraceptive Drugs
Theoretically, peony might interfere with contraceptive drugs due to competition for estrogen receptors.
In vitro and animal research shows that peony extract has estrogenic activity. Concomitant use might also increase the risk for estrogen-related adverse effects.
Cytochrome P450 1A2 (Cyp1A2) Substrates
Theoretically, use of peony may increase the levels and clinical effects of drugs metabolized by CYP1A2.
In vitro research shows that peony suppresses the metabolism of clozapine via weak-to-moderate inhibitory effects on CYP1A2 and CYP3A4. This effect has not been reported in humans.
Cytochrome P450 3A4 (Cyp3A4) Substrates
Theoretically, use of peony may increase the levels and clinical effects of drugs metabolized by CYP3A4.
In vitro research shows that peony suppresses the metabolism of clozapine via weak-to-moderate inhibitory effects on CYP1A2 and CYP3A4. This effect has not been reported in humans.
Estrogens
Theoretically, concomitant use of large amounts of peony might interfere with hormone replacement therapy and/or increase the risk for estrogen-related adverse effects.
In vitro and animal research shows that peony extract has estrogenic activity. Theoretically, peony might compete for estrogen receptors and/or cause additive estrogenic effects.
Phenytoin (Dilantin)
Theoretically, peony might reduce the levels and clinical effects of phenytoin.
Animal research shows that taking peony root reduces levels of phenytoin. Some researchers suggest that peony root might affect cytochrome P450 (CYP) 2C9, which metabolizes phenytoin. However, preliminary research in humans shows that peony root does not alter levels of losartan (Cozaar), which is also metabolized by CYP2C9.
Bai-Zhu Atractylodes (root) extract
Anticoagulant/Antiplatelet Drugs
Theoretically, atractylodes might increase the risk of bleeding when used concomitantly with anticoagulant and antiplatelet drugs.
Laboratory research suggests that atractylenolides II and III, constituents of atractylodes, reduce platelet activation. So far, this has not been shown in humans.
Aromatase Inhibitors
Theoretically, atractylodes may have an additive effect when used with other aromatase inhibitors.
Laboratory research suggests that atractylodes and its constituents exhibit aromatase inhibitor effects.
Hexobarbital
Theoretically, taking atractylodes may prolong the therapeutic and adverse effects of hexobarbital.
In animals, atractylodes has been shown to prolong the effects of hexobarbital. These effects have not been shown in humans.
Cytochrome P450 1A2 (Cyp1A2) Substrates
Theoretically, atractylodes might decrease the levels of CYP1A2 substrates.
In animals, atractylodes administered at high doses has been shown to induce CYP1A2 activity. This effect has not been shown in humans.
Cytochrome P450 3A4 (Cyp3A4) Substrates
Theoretically, atractylodes might increase the levels of CYP3A4 substrates.
In animals, atractylodes administered at high doses has been shown to inhibit CYP3A1 activity, which is a homolog to the human CYP3A4 enzyme. This effect has not been shown in humans.
Momordica charanti (fruit) extract
Antidiabetes Drugs
Taking bitter melon with antidiabetes drugs might increase the risk of hypoglycemia.
Bitter melon can lower blood glucose levels and might have additive effects when used with antidiabetes drugs. This might increase the risk of hypoglycemia in some patients. Monitor blood glucose levels closely.
P-Glycoprotein Substrates
Theoretically, bitter melon might increase levels of P-glycoprotein substrates.
Bitter melon might inhibit the p-glycoprotein (P-gp) intestinal pump and increase intracellular levels of P-gp substrates. In vitro research in intestinal cells shows that 1-monopalmitin, a constituent of bitter melon, increases levels of daunomycin, a P-gp substrate. Additionally, drinking bitter melon juice has been associated with a case of acute pancreatitis in a patient who had been taking pazopanib, a P-gp substrate, for 8 years. Researchers theorize that inhibition of P-gp led to increased levels of pazopanib, resulting in pazopanib-induced pancreatitis.
Pazopanib (Votrient)
Theoretically, bitter melon might increase levels of pazopanib, potentially increasing the risk of adverse effects.
In one case, a 65-year-old patient taking pazopanib for 8 years for renal cell carcinoma experienced signs and symptoms consistent with acute pancreatitis 4 days after drinking bitter melon juice at a dose of 100-150 mL daily. The patient's symptoms, amylase levels, and lipase levels improved upon discontinuation of bitter melon and pazopanib. Pazopanib treatment was re-initiated with no further evidence of pancreatitis. Researchers theorize that inhibition of P-glycoprotein by bitter melon led to increased levels of pazopanib, a P-glycoprotein substrate, resulting in pazopanib-induced pancreatitis.
