In animals, atractylodes administered at high doses has been shown to induce CYP1A2 activity. This effect has not been shown in humans.

This is a very mild, theoretical concern, so I don't want you to lose sleep over it. The idea is that atractylodes might speed up a liver enzyme called CYP1A2, which can help break down certain medicines. If a drug gets cleared faster, you may get a little less benefit from it.

Honestly though, this was only seen in animals given high doses, and it has never been shown in people. Of your three ingredients, none rely heavily on CYP1A2, so any real change in how your cough, congestion, or pain relief works is unlikely. The bigger thing to watch with this combo is keeping your total acetaminophen in check and expecting some drowsiness from the chlorpheniramine. If you take atractylodes regularly, just mention it to your pharmacist.

Theoretically, atractylodes might increase the levels of CYP3A4 substrates.
In animals, atractylodes administered at high doses has been shown to inhibit CYP3A1 activity, which is a homolog to the human CYP3A4 enzyme. This effect has not been shown in humans.

That Vicks Formula 44M combo (acetaminophen for aches and fever, chlorpheniramine for runny nose and sneezing, and dextromethorphan to quiet a cough) is partly handled by a liver enzyme called CYP3A4. The idea here is that atractylodes might slow that enzyme down, which in theory could let these medicines linger a bit longer and bump up their usual effects, like extra drowsiness, dry mouth, or feeling foggy from the dextromethorphan.

Honestly though, this is very weak, theory-only stuff. The slowdown only showed up in animals given high doses, and it has never been proven in people. I'd file it as good to know, not something to lose sleep over. Still, if you take atractylodes regularly, mention it to your pharmacist, and never go over the labeled acetaminophen dose.
