Acetaminophen, Caffeine, Codeine, Salicylamide falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Atractylodes interacts with.

Theoretically, atractylodes might decrease the levels of CYP1A2 substrates.
In animals, atractylodes administered at high doses has been shown to induce CYP1A2 activity. This effect has not been shown in humans.

This combo pill contains caffeine, which your body clears using a liver enzyme called CYP1A2. In animal studies (not people), high doses of atractylodes seemed to speed that enzyme up. If that happened in humans, it could clear caffeine a little faster, meaning the caffeine 'lift' might wear off sooner. Honestly, that is a minor, mostly theoretical concern.

Keep in mind the codeine part works differently. Codeine is a prodrug that depends on a separate enzyme (CYP2D6) to become active, so this particular interaction shouldn't change your pain relief. The acetaminophen and salicylamide aren't really affected here either. Bottom line: low worry, but if you take concentrated atractylodes supplements, mention it to your pharmacist or doctor.

Acetaminophen, Caffeine, Codeine, Salicylamide also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Theoretically, atractylodes might increase the levels of CYP3A4 substrates.
In animals, atractylodes administered at high doses has been shown to inhibit CYP3A1 activity, which is a homolog to the human CYP3A4 enzyme. This effect has not been shown in humans.

Here's the gist for your combo pill (Codalan). The concern on file is that atractylodes might slow down a liver enzyme called CYP3A4, which helps process several of these ingredients. If that happened, the medicine could hang around a little longer and feel a bit stronger. With the codeine piece especially, that could mean more drowsiness, constipation, or feeling lightheaded.

That said, I'd keep this in perspective. The worry comes only from animal and lab studies using high doses, and it has never been shown in people. The interaction is rated minor. Still, since this product contains an opioid, it's worth a quick chat with your pharmacist or doctor before adding atractylodes, just so everyone's on the same page.
