Lactobacillus acidophilus and Bifidobacterium breve are generally well tolerated in healthy children, though they can occasionally cause bloating, flatulence, or gastrointestinal discomfort. Inulin frequently causes gas and bloating, especially at higher doses, and can also cause diarrhea or constipation.

In rare cases, inulin has triggered anaphylaxis in people with inulin sensitivity. The probiotics themselves carry a theoretical but documented risk of infection in severely ill or immunocompromised children — this is uncommon but worth knowing.

L. acidophilus preparations usually contain live and active organisms. Therefore, simultaneously taking antibiotics might kill a significant number of the organisms. Tell patients to separate administration of antibiotics and L. acidophilus preparations by at least two hours.
