Does it work?
Moderate evidence
Coenzyme Q10 is likely effective for treating coenzyme Q10 deficiency itself. For other uses, the evidence is more modest: it's rated possibly effective for fibromyalgia, migraine headaches, congestive heart failure (CHF), and diabetic neuropathy (nerve damage from diabetes).
These "possibly effective" ratings mean the research shows promise, but we don't yet have strong enough data to say it definitely works for those conditions.
How safe is it?
Well-documented data
CoQ10 is generally well tolerated when taken as directed. Gastrointestinal side effects—like stomach upset, diarrhea, nausea, or heartburn—occur in less than 1% of people and can often be reduced by splitting doses above 100 mg throughout the day.
Headache, dizziness, and insomnia have been reported in research. Rarely, skin itching or rash may occur.
There isn't enough reliable safety data in pregnancy, so you should avoid it unless your doctor advises otherwise. Breastfeeding safety hasn't been well studied either, so check with your doctor.

Check with your pharmacist before starting if you take alkylating chemotherapy drugs (like cyclophosphamide)—CoQ10 may reduce their effectiveness. Warfarin (Coumadin) is another important one: CoQ10 might make it less effective at preventing clots.

Kaneka Q10
Alkylating Agents
Coenzyme Q10 has antioxidant effects. Theoretically, this may reduce the activity of chemotherapy drugs that generate free radicals.
Theoretically, antioxidants such as coenzyme Q10 might protect tumor cells from chemotherapeutic agents that work by inducing oxidative stress, such as alkylating agents (e.g., cyclophosphamide) and radiation therapy. The clinical importance of this interaction is unknown.
Warfarin (Coumadin)
Coenzyme Q10 is chemically similar to menaquinone and might have vitamin K-like procoagulant effects, which could decrease the effects of warfarin.
Concomitant use of coenzyme Q10 and warfarin might reduce the anticoagulant effects of warfarin. Four cases of decreased warfarin efficacy thought to be due to coenzyme Q10 have been reported. However, there is some preliminary clinical research that suggests coenzyme Q10 might not significantly decrease the effects of warfarin in patients who have a stable INR.
Antihypertensive Drugs
Theoretically, coenzyme Q10 might have additive effects with antihypertensive drugs.
Some clinical research shows that coenzyme Q10 can significantly lower blood pressure, although other studies have shown conflicting results.
