CoQ10 Ingredients & Drug Interactions
by Zemvelo
What is this page for?
First and foremost: checking CoQ10 against your medications. The heart of this page is the interaction checker and the full interaction report — how this product’s ingredients may interact with prescription and over-the-counter medicines you may be taking.
Around that, we add a pharmacist’s high-level view of the product as a whole — what’s inside, the evidence for its stated use, how transparent the label is, and what safety data exists — so you can see the full picture in one place. It’s educational information from our licensed clinical databases and the clinical staff at HelloPharmacist — not medical advice — and we don’t sell or endorse products. Our editorial policy

Our pharmacy team’s full take, with four database checks built into the cards below — a summary of what is known, not a grade of the product itself.
What’s inside
Full disclosure
Zemvelo CoQ10 is a liquid supplement containing one active ingredient: coenzyme Q10, also called ubiquinone. This is a compound your body naturally makes and stores in cells, where it plays a role in energy production.
The liquid formulation also contains several inactive ingredients—purified water, non-GMO sunflower lecithin, medium chain triglycerides, glycerin, natural flavors, xanthan gum, gum arabic, and potassium sorbate—which help deliver and preserve the CoQ10.
Does it work?
Strong evidence
CoQ10 is rated likely effective for treating CoQ10 deficiency itself. For other conditions, the evidence is more limited: it's rated possibly effective for fibromyalgia, migraine headache, congestive heart failure, and diabetic neuropathy.
This means there's some clinical research supporting its use, but the evidence isn't yet strong or consistent enough to call it proven.
How safe is it?
Well-documented data
CoQ10 is generally well tolerated by most adults when used as directed. In clinical studies, no serious adverse effects have been reported.
The most common side effects are gastrointestinal—appetite suppression, diarrhea, heartburn, nausea, and vomiting—though these occur in less than 1% of users; dividing doses above 100 mg daily may help minimize them. Headache, dizziness, insomnia, and rashes have been reported rarely.
There is not enough reliable safety data during pregnancy, so it's best to avoid use unless your doctor advises otherwise. Safety while breastfeeding has not been well studied, so use caution and check with your doctor.
Meds to double-check
Moderate interaction found
Before taking this CoQ10, double-check your medications for chemotherapy drugs called alkylating agents (such as cyclophosphamide), warfarin or other blood thinners, and blood pressure medications. These have moderate to minor documented interactions that could affect how your drugs work.
The bottom line
Scorecard at a glanceFully disclosed formula with clinical evidence supporting its stated purpose. Moderate medication interactions have been identified, and safety information is well characterized.
CoQ10 may be worth considering if you have a documented CoQ10 deficiency or if you're exploring options for migraine, heart failure, or neuropathy with your doctor's input. If you take warfarin, chemotherapy drugs, or blood pressure medications, you'll need to check with your pharmacist first—CoQ10 can interfere with how these work.
Talk to your pharmacist before starting, especially if you're pregnant or breastfeeding.

CoQ10 by Zemvelo Drug Interactions
HelloPharmacist Interaction Report
Zemvelo CoQ10 is a single-ingredient liquid supplement that interacts with several important medication classes.
The most serious concern is with chemotherapy drugs called alkylating agents (such as cyclophosphamide): CoQ10's antioxidant effect may theoretically reduce how well these cancer medications work by protecting tumor cells from the oxidative stress that makes the drugs effective.
Read the full breakdown — every affected drug type, severity by severity
CoQ10 also has a moderate interaction with warfarin (Coumadin), a blood thinner. Because CoQ10 is chemically similar to vitamin K, it may reduce warfarin's ability to prevent clots.
Four cases of decreased warfarin effectiveness linked to CoQ10 use have been reported. Additionally, CoQ10 may have minor additive effects with blood pressure medications (antihypertensives), which means it could lower your blood pressure further if you're already taking drugs for that purpose — though study results on CoQ10's blood pressure effect itself have been mixed.
Altogether, these interactions span 198 individual medications. Use the medication checker on this page to search your exact prescriptions before starting CoQ10.

For each ingredient in CoQ10 with known interactions, here are the types of medications it can affect. Open any type for the detail — or search your exact drug in the checker above.
CoQ10
Alkylating Agents
Coenzyme Q10 has antioxidant effects. Theoretically, this may reduce the activity of chemotherapy drugs that generate free radicals.
Theoretically, antioxidants such as coenzyme Q10 might protect tumor cells from chemotherapeutic agents that work by inducing oxidative stress, such as alkylating agents (e.g., cyclophosphamide) and radiation therapy. The clinical importance of this interaction is unknown.
Warfarin (Coumadin)
Coenzyme Q10 is chemically similar to menaquinone and might have vitamin K-like procoagulant effects, which could decrease the effects of warfarin.
Concomitant use of coenzyme Q10 and warfarin might reduce the anticoagulant effects of warfarin. Four cases of decreased warfarin efficacy thought to be due to coenzyme Q10 have been reported. However, there is some preliminary clinical research that suggests coenzyme Q10 might not significantly decrease the effects of warfarin in patients who have a stable INR.
Antihypertensive Drugs
Theoretically, coenzyme Q10 might have additive effects with antihypertensive drugs.
Some clinical research shows that coenzyme Q10 can significantly lower blood pressure, although other studies have shown conflicting results.
