Nattokinase is generally well tolerated short-term, but because it thins your blood, it's not suitable if you have a bleeding disorder or high bleeding risk. Rare cases of severe bleeding — including brain bleeding — have been reported, especially when combined with blood thinners.
Nattokinase can trigger allergic reactions in people sensitive to natto (fermented soy), including anaphylaxis in rare cases. Do not use this product during pregnancy — there isn't enough safety data.
