L-glutamine is rated effective for sickle cell disease. For HIV/AIDS-related wasting, postoperative recovery, and critical illness from trauma, the evidence suggests it may help, though the data isn't as strong.
The research we hold shows promise in these areas, but results vary.
How safe is it?
Well-documented data
L-glutamine is generally well tolerated in healthy adults. Common side effects from oral doses include belching, bloating, constipation, diarrhea, flatulence, nausea, vomiting, headache, and gastrointestinal pain — most mild and dose-related.
High doses (10–30 grams daily) used in clinical trials have caused similar digestive issues. Two cases of mania or hypomania were reported in people with bipolar disorder taking glutamine; if you have bipolar disorder, talk to your doctor first.
If you have kidney or liver disease, use it only under medical supervision. There isn't enough safety data to recommend it during pregnancy or breastfeeding unless your doctor advises it — the pregnancy/lactation rating is likely safe, but talk with your clinician for personalized guidance.

Glutamine is metabolized to the excitatory neurotransmitter glutamate. Glutamate might have antagonistic effects with anticonvulsant drugs. However, this interaction has not yet been reported in humans.
