L-Theanine is possibly effective for cognitive function — that's where the evidence is strongest. For age-related cognitive decline, Alzheimer disease, generalized anxiety disorder, obsessive-compulsive disorder, and cancer, the evidence isn't established enough to rate it either way.
If you're considering it for any of those latter uses, there's not reliable data to count on yet.
How safe is it?
Well-documented data
L-Theanine is generally well tolerated in healthy adults for short-term use, but how safe it is long-term hasn't been well studied. The most common side effects reported are headaches, drowsiness, longer sleep, and more vivid dreams.
One pediatric case of facial tics was reported within days of starting 400 mg daily, though the tics stopped when the product was discontinued — and the child had a history of tics, so a clear link wasn't established. There isn't enough safety data to support use during pregnancy or while breastfeeding.

The most serious concern is with blood pressure medications (antihypertensives). Theanine may lower blood pressure on its own, and when combined with drugs that do the same thing, the effect could add up — potentially dropping your pressure too far.
Animal studies suggest this is a real possibility, though we'd want to watch for dizziness or lightheadedness if you're on any blood pressure med.
Theanine may also have minor interactions with sedating drugs (CNS depressants) — theoretically it could add to their drowsy or calming effects, though we're not sure how much this matters in real life. There's also a theoretical minor interaction with serotonergic drugs (antidepressants, certain pain or migraine medications), but clinical evidence of actual serotonin-related problems is absent — studies on theanine's effect on serotonin conflict, and no cases of serotonin syndrome have been reported.

L-Theanine
Antihypertensive Drugs
Theanine might lower blood pressure, potentiating the effects of antihypertensive drugs.
Animal research shows that theanine can lower blood pressure in spontaneously hypertensive animals. Theoretically, concomitant use of theanine and antihypertensive drugs might potentiate the antihypertensive activity.
Cns Depressants
Theoretically, theanine might have additive sedative effects when used in conjunction with CNS depressants. However, it is unclear if this concern is clinically relevant.
Theoretically, theanine may compete with glutamate and/or increase plasma gamma-aminobutyric acid (GABA) levels, which could cause CNS depression. In one clinical study, some subjects taking oral theanine reported drowsiness.
Serotonergic Drugs
Clinical studies regarding the effects of L-theanine on serotonin levels are conflicting. Some studies suggest it can increase serotonin levels in the brain while others report that it may decrease them. Nevertheless, there have been no reports of l-theanine being a causative agent in serotonergic-related side effects or serotonin syndrome.
