Properly prepared elderberry products like this one are generally well tolerated. The concern is with raw or unripe berries, leaves, bark, and seeds, which contain compounds that can cause nausea, vomiting, severe diarrhea, weakness, dizziness, and other effects — but cooking eliminates that risk, and this is a prepared extract.
There's one case report of autoimmune hepatitis in someone taking elderberry for years, and another of liver injury in a patient also taking pazopanib. If you have autoimmune disease or liver concerns, mention this supplement to your doctor or pharmacist.
There is not enough reliable safety data on elderberry during pregnancy or breastfeeding, so it's best to avoid it if you're pregnant or nursing. Talk with your doctor or pharmacist if you're in either situation and thinking about taking it.
