Here's the idea behind this pairing. Elderberry can give your immune system a little nudge, bumping up signals like certain cytokines (including TNF and interleukins). Some medicines are used partly to quiet immune activity, so in theory elderberry could push in the opposite direction and blunt that effect. That's the concern flagged in the database.

The honest catch is that fludrocortisone (Florinef) is mostly used for its mineralocorticoid action, meaning it helps your body hold onto salt and water and keep blood pressure up, often in adrenal insufficiency or low blood pressure on standing. It isn't really prescribed as an immune-suppressing drug, so this theoretical immune effect likely matters very little here. This is lab-based, low-certainty reasoning. Still, before adding elderberry, it's worth a quick check with your pharmacist or doctor.
