Here's the basic idea with elderberry and prednisone. Prednisone works by calming down an overactive immune system, which is exactly why it's used for things like arthritis flares, severe allergic reactions, lupus, and asthma. Elderberry seems to do the opposite in lab studies, gently nudging the immune system to make more inflammatory signals (called cytokines). So in theory, the two could work against each other, and you might not get the full calming benefit you're counting on from your prednisone.

I want to be honest though: this is mostly a theoretical concern based on test-tube and animal data, not strong human proof. Still, if you're on prednisone for a real immune condition, it's worth a quick chat with your pharmacist or doctor before adding elderberry rather than deciding on your own.
