Here's the idea behind this caution. Gamifant (emapalumab) works by calming down an overactive immune system, specifically by blocking a signaling protein called interferon-gamma that drives the dangerous inflammation seen in HLH. Elderberry, on the other hand, has been shown in lab studies to do the opposite. It may nudge the immune system to make more inflammatory messengers like interleukins and tumor necrosis factor.

So in theory, elderberry could push against what your medication is trying to do, possibly making it less effective at controlling the inflammation. I want to be honest though: this is mostly theoretical and based on weak evidence, not real-world cases with this exact drug. Given how serious HLH is, please don't add elderberry without first checking with your doctor or pharmacist, who know your full situation.

Elderberry is a popular herbal supplement, mainly taken to help with colds and flu. Some small studies suggest it may modestly shorten cold or flu symptoms, but the evidence is limited and not conclusive. What Is Emapalumab-lzsg Injection Used For?

Brand name: Gamifant

Gamifant is used to treat primary hemophagocytic lymphohistiocytosis (HLH) — a rare, life-threatening condition where the immune system becomes dangerously overactivated — in adults and children whose disease has not responded to, has come back during, or has progressed on standard HLH therapy, or who cannot tolerate it.

Gamifant is also used for HLH/macrophage activation syndrome (MAS) in adults and children with known or suspected Still's disease, including systemic Juvenile Idiopathic Arthritis (sJIA), when glucocorticoids (steroid medicines) have not worked well enough, cannot be tolerated, or when MAS keeps coming back.
