Nattokinase is generally tolerated when taken short-term, but because it thins the blood, it's not a good fit if you have a bleeding disorder or high bleeding risk. Rare cases of bleeding — including serious brain bleeding — have been reported, especially when combined with blood thinners.

Nattokinase has thrombolytic and anticoagulant effects. There is a case report of intracranial hemorrhage in a patient who added nattokinase to aspirin therapy.
