Nattokinase is generally well tolerated in the short term, but because of its blood-thinning effect, it should be used with caution in anyone at risk of bleeding. Rare reports of bleeding have occurred, including a case of brain bleeding when nattokinase was combined with aspirin.

It can also trigger allergic reactions — including anaphylaxis — in people sensitive to fermented soy or components of natto. There isn't enough safety data to use nattokinase during pregnancy, so it's best avoided.
