Our pharmacy team’s full take, with four database checks built into the cards below — a summary of what is known, not a grade of the product itself.
What’s inside
Full disclosure
This product contains one active ingredient: neem leaf extract in capsule form. Neem is the leaf of a tropical tree, traditionally used in Ayurvedic medicine.
The capsule itself is made from inactive ingredients—hydroxypropyl methylcellulose and purified water—which hold the neem powder together but don't add therapeutic action.
Does it work?
Moderate evidence
Neem leaf has the strongest evidence for gingivitis (gum inflammation) and head lice—both rated possibly effective based on available research. It's also possibly effective for dental plaque.
For acne and tungiasis (a parasitic skin infection), the evidence isn't reliable enough to rate. If you're considering neem for one of those first three uses, talk with your doctor or dentist about whether it's right for you alongside any other treatment you're using.
How safe is it?
Well-documented data
Neem leaf preparations are generally well tolerated when taken by mouth in the short term for most adults, though high-quality safety studies are limited. The main risk is dose and form: neem oil and seed extracts can be toxic if swallowed, and in children, oral neem oil has caused serious harm.
Topically (on the skin), neem is generally well tolerated in both children and adults, though some people with sensitive skin may develop contact dermatitis. Rare serious effects reported with oral neem in adults include heart rhythm problems and kidney damage.
Neem leaf has also caused hemolysis (breakdown of red blood cells) in at least one adult with a specific genetic condition (G6PD deficiency). Because of pregnancy and breastfeeding concerns, neem should be avoided if you are pregnant or nursing—the safety data advises against both.
Meds to double-check
Moderate interaction found
Before taking this product, double-check with your doctor or pharmacist if you take diabetes medications (risk of low blood sugar), immunosuppressants, or any drugs broken down by your liver through the CYP1A2, CYP2C8, CYP2C9, or CYP3A4 pathways, or transported by P-glycoprotein. Most of these interactions are theoretical based on lab work, but they're possible—your pharmacist can tell you whether any of your specific medications fall into these groups.
The bottom line
Scorecard at a glanceFully disclosed formula with some supporting evidence behind its ingredients' uses. Moderate medication interactions have been identified, and safety information is well characterized.
Neem leaf may be worth considering if you have gingivitis, lice, or dental plaque and are looking for a plant-based option. However, if you take diabetes medications, immunosuppressants, or drugs metabolized by certain liver enzymes, you need to clear neem with your doctor or pharmacist first—the interactions could affect how those drugs work.
And if you're pregnant or breastfeeding, skip it. Talk it over with your healthcare provider before starting.

Neem Leaf by Herb Stop Drug Interactions
HelloPharmacist Interaction Report
Herb Stop's Neem Leaf product, containing neem extract, has documented moderate interactions with multiple medication types.
The most serious concern is with antidiabetes drugs—neem can lower blood glucose levels, which means if you're already taking diabetes medication, there's a risk of blood sugar dropping too low (hypoglycemia). Beyond that, neem may affect how your body processes several major drug classes through enzyme and transport inhibition, theoretically raising their levels in your blood.
Read the full breakdown — every affected drug type, severity by severity
The affected medication types include drugs processed through three liver pathways (CYP1A2, CYP2C8, CYP2C9, and CYP3A4 substrates) and those that rely on a cellular pump called P-glycoprotein. Additionally, neem may blunt the effectiveness of immunosuppressant drugs, which are taken to prevent organ rejection or manage certain autoimmune conditions.
Most of these interactions are based on lab research; human cases haven't been reported yet, but that doesn't mean they won't happen.
Altogetherther, these interactions span 1,014 individual medications. Before you start neem, use the medication checker below or talk to your doctor or pharmacist about any prescriptions you take—especially diabetes medications, immune-suppressors, or anything your body breaks down through those liver enzymes.
