Optiflora DI is a 4-ingredient probiotic blend in capsule form. The active ingredients are three strains of live beneficial bacteria — Lactobacillus acidophilus La-14, Lactobacillus plantarum LP-115, Bifidobacterium animalis lactis HN019, and Lactobacillus rhamnosus GG — plus a proprietary blend labeled as supporting digestive and immune function.
The capsule also contains microcrystalline cellulose, hydroxypropyl methylcellulose, and water as inactive ingredients.
Does it work?
Moderate evidence
Lactobacillus acidophilus is rated possibly effective for irritable bowel syndrome (IBS), Helicobacter pylori infection, antibiotic-associated diarrhea, and bacterial vaginosis. It's rated possibly ineffective for atopic disease (allergic conditions), and we don't have enough reliable evidence to rate it for acute respiratory distress syndrome.
We hold no effectiveness data in our monographs for the other three bacterial strains in this product.
How safe is it?
Well-documented data
Lactobacillus acidophilus is generally well tolerated in healthy adults and children. Minor gastrointestinal side effects — epigastric discomfort, abdominal pain, indigestion, gas, bloating, diarrhea, vomiting, and burping — are uncommon when you take it orally.
Because this product contains live microorganisms, there's a theoretical concern that it could cause serious infection (bacteremia, sepsis, abscess, or meningitis) in some patients, though most documented cases appear to stem from bacteria moving from their natural locations in the body rather than from the supplement itself. For pregnancy, the safety data rates Lactobacillus acidophilus as possibly safe, but we don't have pregnancy or breastfeeding ratings for the other three ingredients.

Antibiotic Drugs
Theoretically, taking Lactobacillus acidophilus with antibiotic drugs might decrease the effectiveness of L. acidophilus.
L. acidophilus preparations usually contain live and active organisms. Therefore, simultaneously taking antibiotics might kill a significant number of the organisms. Tell patients to separate administration of antibiotics and L. acidophilus preparations by at least two hours.
