Chamomile is generally well tolerated but can cause allergic reactions in people sensitive to ragweed and related plants, and in rare cases has caused severe hypersensitivity reactions and anaphylaxis. Cascara sagrada may cause mild abdominal cramping with short-term use but carries serious risks with long-term use, including liver damage and loss of muscle mass.

Marshmallow, slippery elm, pumpkin seed, and mullein are generally well tolerated for short-term use, though high-quality safety data are limited for most. Black walnut leaf, bark, and hull contain tannins that may cause gastrointestinal upset, and daily use is associated with an increased risk of tongue cancer and lip leukoplakia.

Violet caused hemolysis (breakdown of red blood cells) in a case report of an infant with G6PD deficiency. During pregnancy, chamomile, marshmallow, cascara sagrada, black walnut, violet, and mullein are best avoided due to insufficient safety data; slippery elm and pumpkin seed have limited data as well.
