Abiraterone falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Cascara Sagrada interacts with.

Theoretically, cascara sagrada might decrease the effects of CYP3A4 substrates.
In vitro research suggests that cascara sagrada can induce CYP3A4 enzymes, albeit to a much lower degree than rifampin, a known CYP3A4 inducer.

Here's the gist: your body uses a liver enzyme called CYP3A4 to help process abiraterone (Zytiga). Lab studies hint that cascara sagrada might gently speed up that enzyme. If that happened in real life, your body could clear the drug a little faster, which in theory could make it work less well at lowering the hormones that feed prostate cancer.

Now the reassuring part. This is based only on test-tube research, and the effect was much weaker than known enzyme boosters like rifampin. So it's more of a theoretical concern than a proven problem. Still, since abiraterone is treating something serious, I'd skip cascara or any stimulant laxative unless your oncologist or pharmacist okays it first.

Cascara sagrada is a stimulant laxative made from the aged bark of a Pacific Northwest tree, used mainly for short-term relief of constipation. Because it can cause cramping, dehydration, and electrolyte problems—and was removed from over-the-counter laxative products in the U.S. due to safety concerns—it should only be used briefly, if at all, and with guidance from a pharmacist or doctor.

About Abiraterone

What Is Abiraterone Used For?

Abiraterone acetate tablets are used to treat two types of advanced prostate cancer: metastatic castration-resistant prostate cancer (CRPC — cancer that has spread and keeps growing despite low testosterone levels) and metastatic high-risk castration-sensitive prostate cancer (CSPC — cancer that has spread but still responds to hormone-lowering treatment). It is always taken together with a corticosteroid — most products pair with prednisone, while Yonsa is specifically indicated with methylprednisolone.

All products — including Abiraterone, Abiraterone Acetate, Abirtega, Yonsa, and Zytiga — are taken by mouth as a tablet. Yonsa is approved only for metastatic CRPC, while the other products (Abiraterone, Abiraterone Acetate, Abirtega, and Zytiga) are approved for both metastatic CRPC and metastatic high-risk CSPC.
