Abemaciclib falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Cascara Sagrada interacts with.

Theoretically, cascara sagrada might decrease the effects of CYP3A4 substrates.
In vitro research suggests that cascara sagrada can induce CYP3A4 enzymes, albeit to a much lower degree than rifampin, a known CYP3A4 inducer.

Here's the situation with Verzenio (abemaciclib) and cascara sagrada. Your body clears abemaciclib mainly through an enzyme called CYP3A4. There's some early lab-dish research hinting that cascara might gently rev up that enzyme. If that happened in a person, it could nudge abemaciclib levels down a bit, which in theory might make your cancer treatment a little less effective. Abemaciclib is not a prodrug, so this points toward slightly weaker effect, not stronger side effects.

I want to be honest with you: this is theoretical and only based on test-tube work, and the effect looked much weaker than known enzyme boosters. So I wouldn't panic. Still, because this is serious cancer therapy, please run any cascara use by your oncologist or pharmacist first.

Cascara sagrada is a stimulant laxative made from the aged bark of a Pacific Northwest tree, used mainly for short-term relief of constipation. Because it can cause cramping, dehydration, and electrolyte problems—and was removed from over-the-counter laxative products in the U.S. due to safety concerns—it should only be used briefly, if at all, and with guidance from a pharmacist or doctor.

About Abemaciclib

What Is Abemaciclib Used For?

Brand name: Verzenio

Verzenio (abemaciclib) is used to treat HR-positive, HER2-negative breast cancer in adults. For early breast cancer at high risk of recurrence, Verzenio is taken alongside endocrine therapy (tamoxifen or an aromatase inhibitor) after surgery to help prevent the cancer from coming back.

For advanced or metastatic breast cancer (cancer that has spread), Verzenio can be used in combination with an aromatase inhibitor as a first hormonal treatment, in combination with fulvestrant after the cancer has progressed on endocrine therapy, or alone after prior endocrine therapy and chemotherapy have stopped working.
