Phosphatidylserine is possibly effective for age-related memory decline and Alzheimer's disease based on the data we hold. For athletic performance, ADHD, overall cognitive function, and dementia, the evidence isn't established — there isn't enough reliable data to say whether it works or doesn't.

How safe is it?
Well-documented data
Phosphatidylserine is generally well tolerated in studies, though long-term safety hasn't been fully worked out. The most common side effects are flatulence, stomach upset, headache, insomnia, and nausea — with insomnia more likely at higher doses (around 600 mg) and stomach upset seen at doses of 200–300 mg per day.
There isn't enough safety information to know whether it's safe during pregnancy, so it's best to avoid it then. Safety while breastfeeding is also unknown, so avoidance is recommended.
