Premier Adrenal Complex contains 8 ingredients, 7 of which are active: cordyceps sinensis mycelia extract, eleuthero, bromelain, burdock, humic acids, rhodiola rosea root extract, and Japanese knotweed root extract. Two of these — Cordy-Pro and Adrenal Pro — are proprietary blends whose individual component ingredients are listed separately above.
The product also contains cellulose as an inactive ingredient (capsule filler). Each active ingredient is included to support adrenal function and energy, though the blend combines traditional adaptogens (cordyceps, eleuthero, rhodiola), proteolytic enzymes (bromelain), and herbal extracts (burdock, Japanese knotweed) with humic acids, a naturally occurring organic compound.
Does it work?
Not established
The evidence for this product's effectiveness is limited across its ingredients. Cordyceps is possibly ineffective for athletic performance and has insufficient evidence for sexual dysfunction, arrhythmia, asthma, lung cancer, and chronic obstructive pulmonary disease.
Eleuthero is possibly effective for genital herpes but has insufficient evidence for familial Mediterranean fever, bipolar disorder, osteoporosis, and chronic fatigue syndrome. Bromelain, burdock, rhodiola, chlorella, and Japanese knotweed all lack sufficient reliable evidence to establish whether they work for their traditional uses — including allergies, burns, cancer, acne, aging skin, altitude sickness, anxiety, depression, and various other conditions listed in their profiles.
The only ingredient with a "possibly effective" rating for a specific condition is eleuthero for genital herpes. For all other claimed benefits in this complex, published evidence is either absent or too weak to confirm they work.
How safe is it?
Well-documented data
Most of these ingredients are generally well tolerated when used short-term, though long-term safety data are limited. Cordyceps may cause abdominal discomfort, constipation, or diarrhea in some people, and there is one rare case report of liver inflammation (cholestatic hepatitis) of unclear causation.
Eleuthero commonly causes diarrhea, indigestion, headache, nausea, and rash; nervousness occurred in about 7% of users in one study, and it may raise blood pressure, especially at higher doses. Bromelain may cause diarrhea, gas, or stomach upset, and inhaling it can trigger allergic reactions including asthma.
Burdock is generally well tolerated as a food but rarely causes allergic reactions including anaphylaxis, and contact dermatitis has been reported with prolonged use. Rhodiola may cause dizziness, dry mouth, or excess saliva.
Chlorella commonly causes gas, cramping, diarrhea, or nausea (especially in the first two weeks), and rare allergic reactions including anaphylaxis have been reported. Japanese knotweed has limited adverse effect data.
All ingredients except chlorella are best avoided during pregnancy and breastfeeding due to insufficient safety data; chlorella is possibly safe in pregnancy but safety during breastfeeding is unknown.

Before taking Premier Adrenal Complex, check with your doctor or pharmacist if you use any of these: blood thinners or antiplatelet drugs (warfarin, aspirin, clopidogrel) — all ingredients raise bleeding risk; immunosuppressants (cyclosporine, tacrolimus); blood sugar medications (metformin, insulin, sulfonylureas); blood pressure medications (especially losartan); heart medications including digoxin (Lanoxin); medications metabolized by liver enzymes including many antidepressants, antihistamines, and pain relievers; or tetracycline antibiotics. These interactions are Moderate to Minor severity depending on the drug and ingredient involved.

The most serious interaction is a Moderate-severity risk of bleeding with anticoagulant or blood thinner drugs (warfarin, aspirin, clopidogrel, and similar medications) — this risk comes from cordyceps, eleuthero, bromelain, burdock, chlorella, and Japanese knotweed together.

The product also carries Moderate-severity interactions with immunosuppressant drugs (such as cyclosporine and tacrolimus) through cordyceps, eleuthero, and rhodiola; with blood sugar medications through eleuthero and rhodiola, raising the risk of low blood sugar; and with drugs that control blood pressure through rhodiola. Eleuthero may raise levels of digoxin (Lanoxin), a heart medication, and could reduce how well warfarin (Coumadin) works by interfering with its mechanism.
Chlorella may also reduce warfarin's effectiveness because it contains vitamin K.
Rhodiola may increase levels of drugs broken down by liver enzymes (CYP2C9, CYP3A4, and CYP1A2 substrates), including losartan (Cozaar) and many others. Japanese knotweed similarly may raise levels of drugs metabolized by several liver pathways (CYP1A2, CYP2C19, CYP3A4, and CYP2E1).
Eleuthero and rhodiola may affect P-glycoprotein substrates — drugs whose levels depend on a cellular transport protein. Bromelain carries a Minor-severity interaction with tetracycline antibiotics.

Rhodiola rosea Root Extract
Antidiabetes Drugs
Theoretically, taking rhodiola with antidiabetes drugs might increase the risk of hypoglycemia.
In vitro and animal research shows that rhodiola extract can decrease blood glucose due to alpha-glucosidase activity.
Antihypertensive Drugs
Theoretically, taking rhodiola with antihypertensive drugs might increase the risk of hypotension.
In vitro and animal research shows that rhodiola extract inhibits angiotensin-converting enzyme (ACE) and might lower blood pressure.
Cytochrome P450 2C9 (Cyp2C9) Substrates
Theoretically, rhodiola might increase levels of drugs metabolized by CYP2C9.
In vitro research shows that rhodiola inhibits CYP2C9. This effect is highly variable and appears to be dependent on the rhodiola product studied. Also, a clinical study in healthy young males found that taking rhodiola extract 290 mg daily for 14 days reduces the metabolism of losartan, a CYP2C9 substrate, by 21% after 4 hours.
Immunosuppressants
Theoretically, rhodiola use might interfere with immunosuppressive therapy.
In vitro and animal research show that rhodiola has immunostimulatory effects.
Losartan (Cozaar)
Rhodiola might increase the levels and adverse effects of losartan.
A clinical study in healthy young males found that taking rhodiola extract 290 mg daily for 14 days reduces the metabolism of losartan, a CYP2C9 substrate, by 21% after 4 hours.
P-Glycoprotein Substrates
Theoretically, rhodiola might increase levels of P-glycoprotein substrates.
In vitro research shows that rhodiola inhibits P-glycoprotein. Theoretically, using rhodiola with P-glycoprotein substrates might increase drug levels and potentially increase the risk of adverse effects.
Antidepressant Drugs
Theoretically, rhodiola might increase the risk of adverse effects when taken with antidepressants.
A review of adverse event reports in Poland identified cases of tachyarrhythmias, myalgia, arthralgia, gum pain, restless leg syndrome, swallowing disorders, and changes in consciousness when rhodiola was taken in combination with paroxetine, escitalopram, fluoxetine, sertraline, trazodone, and/or duloxetine.
Cns Depressants
Theoretically, rhodiola might increase the risk of adverse effects when taken with CNS depressants.
A review of adverse event reports in Poland identified cases of excessive sedation, myoclonus, hypotension, and hallucinations when rhodiola was taken with haloperidol, diazepam, or alprazolam.
Cytochrome P450 1A2 (Cyp1A2) Substrates
Theoretically, rhodiola might increase levels of drugs metabolized by CYP1A2.
In vitro research shows that rhodiola inhibits CYP1A2. This effect is highly variable and appears to be dependent on the rhodiola product studied. However, a clinical study in healthy young males found that taking rhodiola extract 290 mg daily for 14 days does not inhibit the metabolism of caffeine, a CYP1A2 substrate.
Cytochrome P450 3A4 (Cyp3A4) Substrates
Theoretically, rhodiola might increase levels of drugs metabolized by CYP3A4.
In vitro research shows that rhodiola inhibits CYP3A4. This effect is highly variable and appears to be dependent on the rhodiola product studied. However, a clinical study in healthy young males found that taking rhodiola extract 290 mg daily for 14 days does not inhibit the metabolism of midazolam, a CYP3A4 substrate.
Eleuthero
Anticoagulant/Antiplatelet Drugs
Theoretically, eleuthero may have antiplatelet effects and may increase the risk of bleeding if used with anticoagulant or antiplatelet drugs.
In vitro and animal research shows that a constituent of eleuthero, dihydroxybenzoic acid, appears to inhibit platelet aggregation. Concomitant use with anticoagulant or antiplatelet drugs might increase the risk of bleeding. This effect has not been reported in humans.
Antidiabetes Drugs
Theoretically, eleuthero might have additive effects when used with antidiabetes drugs.
Animal research suggests that certain constituents of eleuthero have hypoglycemic activity in both healthy and diabetic animals. A small study in adults with type 2 diabetes also shows that taking eleuthero for 3 months can lower blood glucose levels. However, one very small study in healthy individuals shows that taking powdered eleuthero 3 grams, 40 minutes prior to a 75-gram oral glucose tolerance test, significantly increases postprandial blood glucose levels when compared with placebo. These contradictory findings might be due to patient-specific variability and variability in active ingredient ratios.
Cytochrome P450 1A2 (Cyp1A2) Substrates
Theoretically, eleuthero might increase levels of drugs metabolized by CYP1A2.
In vitro and animal research suggest that standardized extracts of eleuthero inhibit CYP1A2. This effect has not been reported in humans.
Cytochrome P450 2C9 (Cyp2C9) Substrates
Theoretically, eleuthero might increase levels of drugs metabolized by CYP2C9.
In vitro and animal research suggest that standardized extracts of eleuthero might inhibit CYP2C9. This effect has not been reported in humans.
Digoxin (Lanoxin)
Eleuthero might increase serum digoxin levels and increase the risk of side effects.
In one case report, a 74-year-old male who was stabilized on digoxin presented with an elevated serum digoxin level after starting an eleuthero supplement, without symptoms of toxicity. After stopping the supplement, serum digoxin levels returned to normal. It is not clear whether this was due to a pharmacokinetic interaction or to interference with the digoxin assay. Although the product was found to be free of digoxin and digitoxin, it was not tested for other contaminants.
Immunosuppressants
Theoretically, eleuthero might interfere with immunosuppressive drugs because of its immunostimulant activity.
Animal and in vitro research shows that eleuthero extracts have immunomodulatory effects, including increasing cellular and humoral activity.
P-Glycoprotein Substrates
Theoretically, eleuthero might increase levels of P-glycoprotein substrates.
In vitro research suggests that eleuthero can inhibit the multi-drug transporter protein, P-glycoprotein. However, it is too soon to tell if this is clinically important. This interaction has not been reported in humans.
Cytochrome P450 2D6 (Cyp2D6) Substrates
Theoretically, eleuthero might increase levels of drugs metabolized by CYP2D6.
In vitro and animal research suggest that standardized extracts of eleuthero might inhibit CYP2D6. However, research in healthy human volunteers has found that taking eleuthero 485 mg twice daily for 14 days does not inhibit CYP2D6 drug metabolism.
Cytochrome P450 3A4 (Cyp3A4) Substrates
Theoretically, eleuthero might increase levels of drugs metabolized by CYP3A4.
In vitro and animal research suggest that standardized extracts of eleuthero might inhibit CYP3A4. However, research in healthy human volunteers has found that taking eleuthero 485 mg twice daily for 14 days does not inhibit CYP3A4 drug metabolism.
Organic Anion-Transporting Polypeptide Substrates (Oatp)
Theoretically, eleuthero might decrease levels of drugs metabolized by OATP.
In vitro research suggests that eleuthero inhibits OATP2B1, which might reduce the bioavailability of oral drugs that are substrates of OATP2B1. Due to the weak inhibitory effect identified in this study, this interaction is not likely to be clinically significant.
Japanese Knotweed Root Extract
Anticoagulant/Antiplatelet Drugs
Theoretically, hu zhang might increase the risk of bleeding when taken with anticoagulant or antiplatelet drugs.
Hu zhang contains the constituent resveratrol. Resveratrol seems to have antiplatelet effects.
Cytochrome P450 1A2 (Cyp1A2) Substrates
Theoretically, hu zhang might increase levels of drugs metabolized by CYP1A2.

Cytochrome P450 2C19 (Cyp2C19) Substrates
Theoretically, hu zhang might increase levels of drugs metabolized by CYP2C19.
Hu zhang contains the constituent resveratrol. In vitro research shows that resveratrol might inhibit the CYP2C19 enzyme. This interaction has not been reported in humans.
Cytochrome P450 2E1 (Cyp2E1) Substrates
Theoretically, hu zhang might increase levels of drugs metabolized by CYP2E1.
Hu zhang contains the constituent resveratrol. In vitro research shows that resveratrol might inhibit the CYP2E1 enzyme. Also, a pharmacokinetic study shows that taking resveratrol 500 mg daily for 10 days prior to taking a single dose of chlorzoxazone 250 mg increases the maximum concentration of chlorzoxazone by about 54%, the area under the curve of chlorzoxazone by about 72%, and the half-life of chlorzoxazone by about 35%. Chlorzoxazone is used as a probe drug for CYP2E1.
Cytochrome P450 3A4 (Cyp3A4) Substrates
Theoretically, hu zhang might increase levels of drugs metabolized by CYP3A4.
Hu zhang contains the constituent resveratrol. In vitro research shows that resveratrol might inhibit the CYP3A4 enzyme. However, a clinical study in adults with NAFLD found that adding resveratrol 3000 mg daily for 8 weeks did not necessitate dose adjustments to any established medications metabolized by CYP3A4.
Estrogens
Theoretically, hu zhang might competitively inhibit the effects of estrogen replacement therapy.
In vitro research shows that hu zhang might have estrogenic activity.
Carbamazepine (Tegretol)
Theoretically, hu zhang might increase the effects and adverse effects of carbamazepine.
In animals, blood and tissue levels of carbamazepine were increased when given in combination with hu zhang. It is thought that increased levels of carbamazepine are due to cytochrome P450 3A4 (CYP3A4) inhibition. This interaction has not been reported in humans.
Chlorella
Photosensitizing Drugs
Theoretically, chlorella might have additive effects with photosensitizing drugs.
Chlorella has been reported to cause photosensitization. In five case reports, patients who had ingested chlorella exhibited swelling followed by erythematopurpuric lesions on sun-exposed areas of the body. Theoretically, concomitant use with photosensitizing drugs may exacerbate effects.
Warfarin (Coumadin)
Theoretically, chlorella might reduce the clinical effects of warfarin.
Chlorella contains significant amounts of vitamin K. There is at least one case report of warfarin therapy becoming sub-therapeutic after initiation of chlorella supplements.
Cordyceps sinensis mycelia extract
Anticoagulant/Antiplatelet Drugs
Theoretically, cordyceps may increase the risk of bleeding when used with antiplatelet or anticoagulant drugs.
In vitro and animal research suggests that cordyceps extract inhibits platelet aggregation and function. However, this interaction has not been reported in humans.
Immunosuppressants
Theoretically, concurrent use of cordyceps might interfere with immunosuppressive therapy.
Animal and in vitro research suggests that cordyceps stimulates the immune system. However, limited clinical research suggests that taking cordyceps may lower the necessary therapeutic dose of the immunosuppressant cyclosporine, which suggests that cordyceps may have an immunosuppressive effect.
Testosterone
Theoretically, concurrent use of cordyceps and testosterone might have additive effects.
Animal research suggests that cordyceps can increase testosterone levels. The clinical significance of this finding is unclear.
Bromelain
Anticoagulant/Antiplatelet Drugs
Bromelain may have antiplatelet effects and may increase the risk of bleeding if used with anticoagulant or antiplatelet drugs.
There is one case report of a patient experiencing minor bruising while taking bromelain with naproxen. Bromelain is thought to have antiplatelet activity. Whether this interaction is of concern with topical bromelain is unclear. Interference with coagulation of burn wounds has been reported in a patient receiving bromelain-based enzymatic debridement. However, observational research has found that topical bromelain debridement is not associated with increases or decreases in laboratory markers of coagulation when compared with surgical debridement.
Tetracycline Antibiotics
Theoretically, bromelain might increase levels of tetracycline antibiotics.
Laboratory research suggests that bromelain might increase the absorption of tetracycline antibiotics. However, a study in healthy adults reported no difference in tetracycline plasma levels when a 500 mg dose was taken with or without bromelain 80 mg.
Burdock
Anticoagulant/Antiplatelet Drugs
Theoretically, taking burdock with anticoagulant or antiplatelet drugs might increase the risk of bleeding.
In vitro research shows that lignans from burdock reduce rabbit platelet aggregation by inhibiting platelet activating factor. This interaction has not been reported in humans.
