Acalabrutinib falls under P-GLYCOPROTEIN SUBSTRATES — the group Eleuthero interacts with.

Theoretically, eleuthero might increase levels of P-glycoprotein substrates.
In vitro research suggests that eleuthero can inhibit the multi-drug transporter protein, P-glycoprotein. However, it is too soon to tell if this is clinically important. This interaction has not been reported in humans.

Here's the gist for your acalabrutinib (Calquence). Your body uses a kind of cellular bouncer called P-glycoprotein to help move this medicine along and limit how much builds up. In lab dish studies, eleuthero appears to slow that bouncer down. In theory, that could let a little more acalabrutinib stay in your system, which might mean stronger effects or more side effects like bruising, bleeding, headaches, or feeling worn out.

I want to be honest about the evidence, though: this is only test-tube data and has never been reported in real people, so it may not matter at all. Since acalabrutinib treats serious cancers, don't start or stop anything on your own. Just run the eleuthero past your oncologist or pharmacist first.

Theoretically, eleuthero might increase levels of drugs metabolized by CYP3A4.
In vitro and animal research suggest that standardized extracts of eleuthero might inhibit CYP3A4. However, research in healthy human volunteers has found that taking eleuthero 485 mg twice daily for 14 days does not inhibit CYP3A4 drug metabolism.

Here's the thinking behind this pairing. Your body relies on a liver enzyme called CYP3A4 to break down acalabrutinib (Calquence). In lab dishes and animal studies, eleuthero looked like it might slow that enzyme down a bit. If that happened in real life, more acalabrutinib could linger in your system, which in theory could nudge up side effects like bruising, bleeding, headaches, or infections.

The good news: when researchers actually tested eleuthero in healthy people, it did not meaningfully block CYP3A4. So this is mostly a theoretical concern, not something seen in humans. Still, because acalabrutinib treats serious cancers, I'd run any new supplement past your oncologist or pharmacist before starting it.

What Is Acalabrutinib Used For?

Brand name: Calquence

Calquence is used to treat adults with mantle cell lymphoma (MCL) — either newly diagnosed (in combination with bendamustine and rituximab, for patients who are not candidates for a stem cell transplant) or previously treated (after at least one prior therapy). It is also used to treat adults with chronic lymphocytic leukemia (CLL) or small lymphocytic lymphoma (SLL).

Both the capsule and tablet forms of Calquence are approved for all three of these uses. The specific combination regimen and treatment schedule depend on the condition being treated and what other medicines are used alongside it — your care team will map that out for you.
