Eleuthero and Abrocitinib

These two interact in 3 distinct ways. Each one is explained separately below.

Why Are There 3 Interactions?

Eleuthero interacts with whole categories of medication. Abrocitinib falls into 3 of those categories, so it can interact with Eleuthero in 3 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Moderate).

Moderate

CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES

1 of 3

Why this applies to Abrocitinib

Abrocitinib falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES — the group Eleuthero interacts with.

Theoretically, eleuthero might increase levels of drugs metabolized by CYP2C9.
In vitro and animal research suggest that standardized extracts of eleuthero might inhibit CYP2C9. This effect has not been reported in humans.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the gist: your eczema medicine, abrocitinib (Cibinqo), is partly cleared from your body by a liver enzyme called CYP2C9. Lab and animal studies hint that eleuthero might slow that enzyme down. If that happened in people, abrocitinib could build up a bit more than usual, which in theory could mean stronger effects and a higher chance of side effects like nausea, headache, acne, or a greater risk of infection from a more suppressed immune system.

The important part is that this is theoretical only. It has been seen in test tubes and animals, never confirmed in humans, so I wouldn't lose sleep over it. Still, since abrocitinib already needs careful monitoring, it's worth mentioning eleuthero to your pharmacist or doctor before combining them.

ANTICOAGULANT/ANTIPLATELET DRUGS

2 of 3

Abrocitinib also falls under ANTICOAGULANT/ANTIPLATELET DRUGS.

Theoretically, eleuthero may have antiplatelet effects and may increase the risk of bleeding if used with anticoagulant or antiplatelet drugs.
In vitro and animal research shows that a constituent of eleuthero, dihydroxybenzoic acid, appears to inhibit platelet aggregation. Concomitant use with anticoagulant or antiplatelet drugs might increase the risk of bleeding. This effect has not been reported in humans.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS

Here is something worth knowing about pairing eleuthero with abrocitinib (Cibinqo). On its own, abrocitinib is a JAK inhibitor for eczema, and one of its known side effects is a small effect on platelets, the cells that help your blood clot. Lab and animal studies suggest a compound in eleuthero may also make platelets a little less sticky.

Put together, the theory is that the two could nudge your bleeding risk up a bit, showing as easier bruising, nosebleeds, or cuts that take longer to stop. I want to be honest, though: this has only been seen in test tubes and animals, never reported in people, so the evidence is weak. Please don't change anything on your own. Just give your pharmacist or doctor a heads up before adding eleuthero, especially if you notice any unusual bruising or bleeding.

IMMUNOSUPPRESSANTS

3 of 3

Abrocitinib also falls under IMMUNOSUPPRESSANTS.

Theoretically, eleuthero might interfere with immunosuppressive drugs because of its immunostimulant activity.
Animal and in vitro research shows that eleuthero extracts have immunomodulatory effects, including increasing cellular and humoral activity.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Here's the thinking behind this pairing. Abrocitinib (Cibinqo) works by dialing down an overactive immune system to calm your eczema. Eleuthero, on the other hand, is traditionally used as an immune 'booster.' So in theory, the herb could push your immune activity in the opposite direction from what your medication is trying to do, potentially making Cibinqo a little less effective and letting that dry, itchy, scaly skin creep back.

I want to be honest with you, though: this concern comes only from animal and lab studies, not from people taking these two together. It is more of a 'just in case' caution than a proven problem. Since keeping your eczema controlled matters, it's worth a quick chat with your pharmacist or dermatologist before adding eleuthero rather than starting it on your own.

From our pharmacist’s overview on the Abrocitinib page

“Bleeding is where I'd focus first with abrocitinib (Cibinqo), an eczema pill that can itself lower platelet counts. Of the 290 supplements and herbs with an interaction on file, 202 have blood-thinning or antiplatelet activity, and two of the three major-rated entries, danshen and piracetam, sit in that group.”

About Eleuthero

What is Eleuthero?

Eleuthero
  • Fatigue and low energy
  • Physical and athletic performance
  • Stress resistance (adaptogen)
  • Immune support
  • Mental focus and alertness

Eleuthero is an herb traditionally used as an 'adaptogen' to fight fatigue, boost energy, and help the body handle stress. The scientific evidence behind these uses is limited and mixed, so it should not replace proven treatments. It is generally well tolerated short-term, but pregnant and breastfeeding people and those with certain conditions should avoid it or check with a healthcare provider first.
