Bromelain is generally well tolerated short-term but may cause diarrhea, gas, stomach upset, or headache, and is best avoided during pregnancy and breastfeeding since safety data are lacking.
The digestive enzymes (protease, lipase, phytase, fructooligosaccharides) may cause digestive upset or gas and bloating, especially at higher doses. The probiotics are generally well tolerated in healthy adults and children, though they may rarely cause gastrointestinal side effects (bloating, gas, abdominal pain, diarrhea) and very rarely have been reported in cases of infection in vulnerable patients — for instance, Lactobacillus acidophilus has been isolated in rare cases of bacteremia and sepsis, though usually in people with other risk factors.

The most serious concern is papain with warfarin (Coumadin), a blood thinner — papain may increase warfarin's effects and side effects, and one case report documented a dangerous rise in blood-thinning levels (INR of 7.4) when a patient on stable warfarin took a papain-containing supplement.

Bromelain may increase bleeding risk if you take anticoagulants (blood thinners) or antiplatelet drugs (like aspirin or clopidogrel), since it has antiplatelet properties. Bromelain may also theoretically raise levels of tetracycline antibiotics, though one study in healthy adults found no difference in tetracycline blood levels when bromelain was added.
All three probiotic strains — the two Lactobacillus acidophilus varieties and both Bifidobacterium species — may lose effectiveness if taken at the same time as antibiotics, since the drugs can kill the live organisms in the supplement. Altogether, these interactions span 306 individual medications.

Lactobacillus acidophilus (CUL 60)
Antibiotic Drugs
Theoretically, taking Lactobacillus acidophilus with antibiotic drugs might decrease the effectiveness of L. acidophilus.
L. acidophilus preparations usually contain live and active organisms. Therefore, simultaneously taking antibiotics might kill a significant number of the organisms. Tell patients to separate administration of antibiotics and L. acidophilus preparations by at least two hours.
Bifidobacterium bifidum (CUL 20)
Antibiotic Drugs
Theoretically, taking Bifidobacterium. bifidum with antibiotic drugs might decrease the effectiveness of B. bifidum.
Since B. bifidum preparations usually contain live and active organisms, simultaneously taking antibiotics might kill a significant number of the organisms. Tell patients to separate administration of antibiotics and B. bifidum preparations by at least 2 hours.
Bifidobacterium animalis subsp. lactis (CUL 34)
Antibiotic Drugs
Theoretically, taking Bifidobacterium lactis with antibiotic drugs might decrease the effectiveness of B. lactis.
Since B. lactis preparations usually contain live and active organisms, simultaneously taking antibiotics might kill a significant number of the organisms. Tell patients to separate administration of antibiotics and B. lactis preparations by at least 2 hours.
Bromelain
Anticoagulant/Antiplatelet Drugs
Bromelain may have antiplatelet effects and may increase the risk of bleeding if used with anticoagulant or antiplatelet drugs.
There is one case report of a patient experiencing minor bruising while taking bromelain with naproxen. Bromelain is thought to have antiplatelet activity. Whether this interaction is of concern with topical bromelain is unclear. Interference with coagulation of burn wounds has been reported in a patient receiving bromelain-based enzymatic debridement. However, observational research has found that topical bromelain debridement is not associated with increases or decreases in laboratory markers of coagulation when compared with surgical debridement.
Tetracycline Antibiotics
Theoretically, bromelain might increase levels of tetracycline antibiotics.
Laboratory research suggests that bromelain might increase the absorption of tetracycline antibiotics. However, a study in healthy adults reported no difference in tetracycline plasma levels when a 500 mg dose was taken with or without bromelain 80 mg.
Papain
Warfarin (Coumadin)
Theoretically, papain might increase the effects and side effects of warfarin.
In one case report, a patient previously stable on warfarin was found to have an international normalization ratio (INR) of 7.4, which was attributed to ingestion of a supplement containing papain from papaya extract.
