Probiotic Pearls Immune contains 5 active ingredients — all live bacterial cultures designed to support digestive and immune health. Lactoferrin is a protein found naturally in milk that your body uses to help with iron absorption and immune function.
The other four are probiotic bacteria: Lactobacillus plantarum Lp-115, Bifidobacterium lactis Bl-04, Lactobacillus acidophilus NCFM, and Bifidobacterium longum BB536. These "good" bacteria are meant to help maintain a healthy gut environment.
The softgel capsule also contains inactive ingredients — fish gelatin (the capsule shell), plus palm oil, glycerin, soy lecithin, silica, and pectin as binders and fillers.
Does it work?
Moderate evidence
Lactoferrin shows possibly effective evidence for sepsis and pregnancy-related iron deficiency, but the data show it's likely ineffective for reducing fetal and premature infant death or for necrotizing enterocolitis (a serious bowel condition in newborns). Lactobacillus acidophilus is possibly effective for irritable bowel syndrome, Helicobacter pylori infection, antibiotic-associated diarrhea, and bacterial vaginosis.
For Bifidobacterium longum, the evidence we hold is insufficient to rate it for the conditions studied — including antibiotic-associated diarrhea, hay fever, anxiety, asthma, and athletic performance. The other two bacterial strains (Lactobacillus plantarum and Bifidobacterium lactis) have no effectiveness ratings in our data.
How safe is it?
Well-documented data
Lactoferrin is rated likely safe in pregnancy and possibly safe when used as a supplement; lactation is rated likely safe, though long-term safety data are limited and the safety of supplements while breastfeeding hasn't been well studied — use only under medical supervision in pregnancy and discuss breastfeeding use with your healthcare provider. At typical doses, lactoferrin is well tolerated, but doses of 7.2 grams daily or higher have been linked to constipation, diarrhea, anorexia, fatigue, and chills.
Lactobacillus acidophilus is generally well tolerated in healthy adults and children and is rated possibly safe in pregnancy; its lactation rating is incomplete in our data. Rare cases of serious infection (bacteremia, sepsis, abscess) have been reported, though most occur in hospitalized or vulnerable patients.
Mild gastrointestinal side effects like bloating, flatulence, and diarrhea can occur but are uncommon. Bifidobacterium longum is rated possibly safe for both pregnancy and lactation but carries limited safety data — check with your doctor before use, especially if you're seriously ill or have a weakened immune system.
Rare bacteremia and sepsis cases have occurred in critically ill infants; mild abdominal discomfort and flatulence are rare.

HOWARU L. acidophilus NCFM
Antibiotic Drugs
Theoretically, taking Lactobacillus acidophilus with antibiotic drugs might decrease the effectiveness of L. acidophilus.
L. acidophilus preparations usually contain live and active organisms. Therefore, simultaneously taking antibiotics might kill a significant number of the organisms. Tell patients to separate administration of antibiotics and L. acidophilus preparations by at least two hours.
Bifidobacterium longum BB536
Antibiotic Drugs
Theoretically, taking Bifidobacterium longum with antibiotic drugs might decrease the effectiveness of B. longum.
Since B. longum preparations usually contain live and active organisms, simultaneously taking antibiotics might kill a significant number of the organisms. Tell patients to separate administration of antibiotics and B. longum preparations by at least 2 hours.
