The evidence for most conditions is limited. Lactobacillus acidophilus shows possibly effective ratings for irritable bowel syndrome, Helicobacter pylori infection, antibiotic-associated diarrhea, and bacterial vaginosis.
Bifidobacterium bifidum is possibly effective for irritable bowel syndrome and respiratory tract infections. For most other conditions the facts list — allergic rhinitis, asthma, cognitive decline, anxiety — the evidence isn't established in the data we hold.
How safe is it?

These probiotics are generally well tolerated in healthy adults and children. However, caution is advised if you're seriously ill, have a weakened immune system, or are in the hospital — there are rare reports of Bifidobacterium and Lactobacillus bacteremia (bacteria in the bloodstream) in critically ill patients, particularly preterm infants.
Mild gastrointestinal side effects like bloating, flatulence, abdominal discomfort, and diarrhea have been reported rarely. Lactobacillus acidophilus is rated possibly safe in pregnancy; Bifidobacterium longum and Bifidobacterium bifidum are also rated possibly safe in pregnancy.
For breastfeeding, limited data exist, so check with your doctor or pharmacist before use.

Lactobacillus acidophilus

Theoretically, taking Lactobacillus acidophilus with antibiotic drugs might decrease the effectiveness of L. acidophilus.
L. acidophilus preparations usually contain live and active organisms. Therefore, simultaneously taking antibiotics might kill a significant number of the organisms. Tell patients to separate administration of antibiotics and L. acidophilus preparations by at least two hours.
