This product contains four active ingredients. Bacillus subtilis is a live microorganism used to support digestive health.
Livaux is a standardized extract from kiwi fruit. Spirulina is a type of blue-green algae.
We hold no interaction data for the fourth ingredient, Ayuric. The remaining ingredients are fillers and capsule material: hypromellose, rice powder, and rice hull concentrate.
Does it work?
Not established
Bacillus subtilis shows possibly effective evidence for antibiotic-associated diarrhea, but evidence is insufficient to rate it for athletic performance, cirrhosis, constipation, eczema, or diabetes. Livaux (kiwi) has insufficient evidence to rate for asthma, constipation, high blood pressure, or irritable bowel syndrome.
Spirulina shows possibly effective evidence for high blood pressure, but evidence is insufficient to rate it for acne, allergic rhinitis (hay fever), iron-deficiency anemia, or arsenic poisoning. No effectiveness data is on file for Ayuric.
How safe is it?
Well-documented data
Bacillus subtilis is generally well tolerated in healthy adults, though quality and safety vary by product and strain. There is not enough safety data overall, so it's best to avoid it unless your doctor advises otherwise.
One person reported a headache in a clinical trial, and diarrhea has been reported rarely. Livaux (kiwi) as a food is generally safe, but allergic reactions are possible—symptoms can range from tingling or swelling of the lips and mouth to vomiting, hives, and in rare cases anaphylaxis or asthma flare-ups.
Spirulina blue-green algae is generally well tolerated, but quality varies and some products can be contaminated with toxins or heavy metals; safety is not well studied and contamination is a concern. Reported side effects include stomach cramps, bloating, diarrhea, dizziness, fatigue, gas, headache, nausea, vomiting, and rashes.
There is not enough safety data on Bacillus subtilis or Ayuric in pregnancy and lactation to know whether they are safe; talk with your doctor or pharmacist for personalized advice. Livaux (kiwi) is likely safe in pregnancy and lactation.

Alerna Kidney Health Prostate Support interacts with medications through its Bacillus subtilis, Livaux (kiwi), and spirulina content.
The most serious interaction is a Moderate-severity risk of low blood pressure (hypotension) when kiwi is combined with blood pressure–lowering drugs (antihypertensive drugs) like captopril or enalapril. Both kiwi and spirulina carry Moderate-severity risks of increased bleeding when taken with blood thinners or antiplatelet drugs such as warfarin, aspirin, or clopidogrel.
Spirulina also presents Moderate-severity risks with drugs that suppress the immune system (immunosuppressants) and with diabetes medications, since the ingredient may lower blood sugar (hypoglycemia). Bacillus subtilis may reduce its own effectiveness if you take antibiotics at the same time, a Moderate-severity concern that's best managed by spacing the two apart.

Theoretically, spirulina blue-green algae might increase the risk of bleeding if used with other anticoagulant or antiplatelet drugs. However, this is unlikely.
Spirulina blue-green algae have shown antiplatelet and anticoagulant effects in vitro. However, one preliminary study in 24 patients receiving spirulina blue-green algae 2.3 grams daily for 2 weeks showed no effect on platelet activation or measures of clotting time.
Antidiabetes Drugs
Theoretically, taking blue-green algae with antidiabetes drugs might increase the risk of hypoglycemia.
Human research shows that spirulina blue-green algae can have hypoglycemic effects in patients with diabetes, at least some of whom were using antidiabetes drugs. However, blue-green algae does not seem to improve glycated hemoglobin (HbA1c) levels in patients with diabetes. A meta-analysis of animal studies also suggests that spirulina blue-green algae have hypoglycemic effects.
Immunosuppressants
Theoretically, concurrent use of blue-green algae might interfere with immunosuppressive therapy.
Blue-green algae have been shown to stimulate the immune system.

Anticoagulant/Antiplatelet Drugs
Clinical research suggests that kiwi inhibits platelet aggregation. Theoretically, kiwi might increase the risk of bleeding when used with antiplatelet or anticoagulant drugs. Some anticoagulant or antiplatelet drugs include aspirin, clopidogrel (Plavix), dalteparin (Fragmin), enoxaparin (Lovenox), heparin, ticlopidine (Ticlid), warfarin (Coumadin), and others.
Antihypertensive Drugs
Clinical research suggests that consuming kiwi reduces systolic and diastolic blood pressure in hypertensive individuals. Theoretically, concomitant use of kiwi and antihypertensive drugs may increase the risk of hypotension when used in combination with drugs that lower blood pressure. These include captopril (Capoten), enalapril (Vasotec), losartan (Cozaar), valsartan (Diovan), diltiazem (Cardizem), amlodipine (Norvasc), hydrochlorothiazide (HydroDiuril), furosemide (Lasix), and many others.
Bacillus subtilis
Antibiotic Drugs
Theoretically, taking Bacillus subtilis with antibiotic drugs might decrease the effectiveness of B. subtilis.
Since B. subtilis preparations usually contain live and active organisms, simultaneously taking antibiotics might kill a significant number of the organisms. Tell patients to separate administration of antibiotics and B. subtilis preparations by at least 2 hours.
